JAHA:收缩压和动脉僵硬度进展之间的关系

基线收缩压与动脉僵硬度进展有关,与年龄无关。

动脉僵硬度可预测血管事件和全因死亡的风险,并与年龄和高血压相关。但是,血压(BP)与动脉僵硬程度之间关系的大小尚未明确。

近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,研究人员检索了Medline和EMBASE数据库,确定了报告基线BP与动脉僵硬度进展之间线性模型的前瞻性研究,并根据人口统计学特征和基线动脉僵硬度进行调整和未调整分析。研究人员通过固定和随机效应荟萃分析将脉搏波速度的标准化和非标准化β系数进行合并,并按反方差加权。

研究人员从30524个标题中确定了566篇文献进行全面审查,纳入了21项研究中报告的22个人群。在9个队列中,基线收缩压与动脉僵硬进展之间的关联一致(11781名患者;标准β=0.041;95%CI为0.026-0.055;P<0.001;异质性P值=0.70),相当于收缩压每10年降低20mmHg,标准颈动脉-股动脉脉搏波速度增加1.14m/s。调整后的关联相似(1762名患者;β=0.0047;95%CI为0.004-0.006;P<0.001;异质性的P=0.64),相当于收缩压每10年降低20mmHg,标准颈动脉-股动脉脉搏波速度增加0.94m/s。在有限的研究中,平均BP与动脉僵硬度进展之间的标准化关联并不显著,且存在异质性(913名患者;β=0.039;95%CI为-0.008至0.086;P=0.11;异质性P=0.03)。

由此可见,基线收缩压与动脉僵硬度进展有关,与年龄无关。
