PD患者的RLS患病率高于预期

近日研究人员就不安腿综合征（RLS）和运动障碍（包括帕金森病（PD）、其他帕金森综合征、原发性震颤、舞蹈和肌张力障碍综合征、Tourette综合征和遗传性共济失调）之间的联系进行了考察。

研究人员对1966年至2018年9月PubMed发表的文章进行检索，采用meta-disc1.1.1软件和prisma指南对PD患者和对照组的RLS频率进行了荟萃分析。

虽然RLS和PD在临床、神经影像学、神经病理和遗传等方面有很大的差异，但许多报告表明，与一般人群或与之匹配的对照组相比，PD患者的RLS患病率高于预期，也有一些研究表明，RLS可能是PD的早期临床特征。RLS症状多见于多系统萎缩、原发性震颤、Tourette综合征、Friedreich共济失调和III型脊髓小脑共济失调。最后，PD和RLS (α-syNucin基因启动子中复杂微卫星重复Rep1等位基因2)；Tourette综合征与RLS(BTBD 9基因变异)之间的可能存在遗传相关性，但上述数据是基于小样本的初步数据，需要在进一步验证。

然而，在多巴胺受体激动剂长期治疗过程中，多巴胺增强和症状的严重程度的整体增加同时发生导致在初始治疗中转向非多巴胺能的替代品，特别是α2δ配体。新的国际指南推荐，如果可能的话，使用这些药物（α2δ配体）进行初始治疗可以避免从开始就出现多巴胺增强。其他（如谷氨酸能或腺苷）神经递质也可能在导致不安腿综合征/WED中发挥重要作用，因此可能出现新的治疗方法。
