改善生活方式可降压、调脂和降糖，效果类似于吃药！欧洲心脏学会教育课程

国际上所有的指南都强调生活方式改变对于心血管病预防非常重要，可在药物治疗前或同时进行生活方式调节。但实际上，人群中健康生活方式的依从率远远未达到理想状态。

肥胖、高血压、吸烟、血脂代谢异常、糖尿病等都是可调节的心血管病危险因素。国际上所有的指南都强调生活方式改变对于心血管病预防非常重要，可在药物治疗前或同时进行生活方式调节。但实际上，人群中健康生活方式的依从率远远未达到理想状态。
 
近日，Heart杂志在线发表的一篇文章汇总分析了生活方式改变与药物治疗对各种主要心血管病危险因素的疗效。这篇文章是欧洲心脏学会教育课程的两个章节。
 
分析显示，充分改变生活方式对心血管病危险因素的降低效果，接近于单个药物治疗。

在降压方面，总体上，改善生活方式的降压效果近似于用一种降压药物，但比不上联合使用降压药物。
 
但理论上，遵循戒烟、健康饮食、有氧运动等多个健康生活方式可将收缩压和舒张压分别降低10 mmHg和8 mmHg，效果接近或等同于某些降压药物联用时的疗效。
 
减肥、减少盐和糖的摄入、增加钾和纤维摄入、加强有氧运动均可降低血压。

在调脂方面，健康饮食可能比戒烟或运动更有效。
 
健康饮食可降低总胆固醇和坏胆固醇（低密度脂蛋白胆固醇）水平。
 
减肥主要降低坏胆固醇水平；戒烟可升高好胆固醇（高密度脂蛋白胆固醇）水平。
 
在多重生活方式干预下，饱和脂肪酸摄入量减少10%、膳食纤维增加15~20 g、体重减轻5 kg、富含植物甾醇食物摄入增加的情况下，坏胆固醇水平最高降幅可达35%，其效果略低于他汀单药治疗。
 
在降低糖化血红蛋白水平上，运动可能比饮食更有效，这在糖尿病患者中比较明显。
 
生活方式干预对糖化血红蛋白的降低效果与某些降糖药物接近。
 
但如果戒烟后体重增加，会导致血糖和糖化血红蛋白水平升高。
 
作者强调，生活方式调节应该贯穿于人的一生。尤其要注意的是，即便已经接受药物治疗，也不能放松对生活方式的干预。
