J Urol:前列腺癌患者中,手术去势和药物去势的心脏毒性比较

比较了前列腺癌患者接受手术去势或GnRH疗法时,发生重大不良心脑血管事件(MACCEs)的风险情况。

雄性激素剥夺疗法(ADT)包括双侧睾丸切除术和长效促性腺激素释放激素(GnRH)激动剂/拮抗剂。关于ADT相关的心血管结局仍有争议。

近期,有研究人员在《J Urol》杂志上发表文章,比较了前列腺癌患者接受手术去势或GnRH疗法时,发生重大不良心脑血管事件(MACCEs)的风险情况

研究人员使用了利用癌症登记处(TCR)和健康保险研究数据库(NHIRD)中的数据,鉴定了2008年-2017年接受GnRH疗法的8413名患者和接受外科去势的694名患者。排除的标准是癌症阶段医疗记录不完整,以及年龄小于18岁。主要结果是MACCEs的复合终点,其中包括手术去势或药物去势之后的急性心肌梗死(AMI)、需要住院的充血性心力衰竭(CHF)和缺血性中风(包括短暂性脑缺血发作[TIA])。随访时间的中位数为3年。采用多变量Cox比例风险模型调查了终点和不同治疗方法之间的关系,并对年龄、临床阶段、合并症、药物和诊断的癌症年份进行了调整。

研究结果发现,在接受GnRH疗法或手术去势的患者中,3年死亡率和MACCEs的粗发生率分别为19.90% vs 26.51%和8.23% vs 8.65%。调整了年龄、癌症分期和合并症后,结果发现组间MACCEs没有显著差异,但与接受GnRH疗法的患者相比,接受手术去势患者的急性心肌梗死(AMI)发生率略有增加。接受手术去势患者的MACCEs和AMI的调整后死亡率危险比比接受GnRH疗法的患者分别高1.11倍和1.8倍。值得注意的是,在有关癌症分期的亚组分析中,与GnRH疗法相比,接受手术去势的患者中,癌症分期为IV患者的AMI风险增加最明显。

手术去势或者药物去势后的3年MACCEs风险亚组分析

综上所述,前列腺癌患者发生AMI的风险增加,尤其是接受手术去势的患者,而非接受GnRH疗法的患者。该研究也强调了应该关注手术去势与GnRH疗法对心脏安全性的影响
