【1】Int Arch Otorhinolaryngol:耳蜗植入前评估:重度双侧舌前听力损失儿童的临床资料和家族史

巴基斯坦的耳聋患病率高。对深度听力损失患者的临床特征的了解不仅有助于查明原因,也有助于公共卫生干预措施的策略规划。

最近,有研究人员调查了听力损失儿童的临床情况,即发病年龄、相关缺陷和障碍、可能的病因、相关家族史以及近亲结婚的影响。研究自2016年11月至2018年9月进行。研究人员将所有6岁以下的双侧深度听力损失、且将会受益于人工耳蜗植入术的儿童患者纳入了研究。研究详细记录了病史,并对患者各方面的发育能力进行了评估。根据Schedule of Growing Skills II对患者的运动能力、操作能力、视觉能力、语言能力、社交能力和自理能力进行评分。研究人员还对患儿父母进行了详细的家族史调查。研究表明,待治疗患儿的平均年龄为3.2±1.25岁。大多数患者(51.5%)有阳性家族病史。近亲结婚很常见;76.9%的患者的父母是一级亲属。大多数患者(90.8%)有相关语言障碍。总共有4名(?3.07%)患者有全局性发育迟缓。

最后,研究人员指出,近亲结婚在该疾病的发展过程中具有重要作用。这些儿童的发育与会诊的年龄紧密相关。

【2】Ear Nose Throat J:治疗慢性中耳炎手术术后感音神经性听力损失发病率调查

最近,有研究人员调查了慢性中耳炎(COM)手术后感音神经性听力损失(SNHL)的发生率,并确定了相关因素。

研究人员通过回顾性病历审查收集数据。在192名患者中,82名接受了鼓膜成形术,26名接受了鼓膜成形术的管壁上乳突切除术,84名接受了鼓膜成形术的管壁下乳突切除术。术后,平均空气传导(AC)听力阈值从125-1000 Hz明显改善,但高频AC均值和骨传导(BC)听力阈值明显恶化。在21(11%)个案例中,BC听力阈值在4000Hz时恶化了15dB以上。当研究人员将这21个案例与听力得以保留的患者进行比较时,发现前者的年龄明显偏小,且胆脂瘤性中耳炎的发生频率更高。然而,颞骨计算机断层扫描患者的炎症严重程度时,两组之间无明显差异。

最后,研究人员指出,高频SNHL可能在手术治疗COM后出现,尤其是年轻的胆脂瘤患者。

【3】Laryngoscope:助听器或人工耳蜗对听力障碍的长期影响

之前的研究表明,在中短期随访中,助听器的使用或人工耳蜗植入后听力障碍会有所减轻,然而听力损失干预措施对听力障碍长期的影响仍然研究不足。最近,有研究人员报道了助听器使用或人工耳蜗植入后6个月、12个月和5年的听力障碍情况。

研究是一个观察性研究,包括了115名来自 "听觉康复治疗后的多种结果研究"(SMART)研究队列的参与者。研究结果发现,与基线相比,人群平均HHIE-S评分随时间变化的相关性,考虑到重复测量数据的相关性,用链式方程进行多重归纳,归纳缺失数据。与基线相比,助听器使用后的平均HHIE-S得分在第6个月(均值=-7.96,95%CI:-10.40,-5.53)、12个月(均值=-6.58,95%CI:-9.26,-3.90)和5年(均值=-4.58,95%CI:-7.87,-1.30)均显著降低。人工耳蜗植入后,6个月(均值=-8.18,95%CI:-11.07,-5.30)、12个月(均值=-10.04,95%CI:-12.92,-7.16)和5年(均值=-8.97,95%CI:-12.92,-7.16)时听力障碍评分均值较基线也显著降低。

最后,研究人员指出,他们的研究发现助听器和人工耳蜗植入术对听力障碍的短期益处能够在5年内得以维持。

【4】Int J Pediatr Otorhinolaryngol:先天性巨细胞病毒感染儿童的人工耳蜗植入结果

到目前为止,巨细胞病毒(cCMV)是儿童永久性感音神经性听力损失(SNHL)最常见的非遗传性先天性病因,也是造成发育和神经系统残疾的主要感染性原因。最近,有研究人员阐释了由有症状或无症状的cCMV感染而导致重度至深度感音神经性听力损失的儿童的人工耳蜗植入结果,特别是比较了其与匹配的突变Connexin 26(Cx26)植入患者的表现结果。

研究是一个回顾性病例对照研究。研究人员收集了有症状cCMV和无症状cCMV患者的临床资料,并与年龄和CI前(人工耳蜗植入前)语言类别相匹配的Cx26患者的临床资料进行了比较;所有受试者均受双侧重度至深度SNHL影响,并接受CI和语言康复治疗。结果表明,统计学分析显示三个小组CI前感知类别和语言水平无显著差异,有症状cCMV组语言阶段表现随时间推移显著更差(P=0.017)。另外,他们的数据支持了受cCMV影响的儿童的语言能力能够随着时间的推移而改善,尽管与无症状的cCMV和Cx26突变组相比,有症状的cCMV组在CI后3-4年取得的语言阶段较低。

最后,研究人员指出,到目前为止,CI辅以言语治疗可以认为是对cCMV相关的重度至深度听力损失儿童的有效干预措施。

【5】JAMA Otolaryngol Head Neck Surg:单侧耳聋儿童的耳蜗植入治疗

最近,有研究人员评估了因单侧耳聋(SSD)而接受人工耳蜗植入的儿童的听觉和患者报告的结果情况,并评估了植入时间、主观结果和人工耳蜗装置使用率之间的关系。

研究包括了12项观察性研究,共评估了119名接受人工耳蜗植入的SSD儿童(平均[SD]年龄:6.6[4.0]岁)。结果表明,大多数儿童在噪音(49名儿童中的39名[79.6%])和安静(42名儿童中的34名[81.0%])中的语言感知具有临床意义的改善。长时间的耳聋(先天性SSD时间超过4年,语前SSD时间超过7年)是最常见的没有改善原因。人工耳蜗植入后,通过与真实位置的误差度测量的声音定位(平均差值[MD],-24.78°;95%CI,-34.16°至-15.40°;I2 = 10%)在统计学上有显著改善。与先天性SSD患者相比,后天性SSD患者和耳聋持续时间较短的患者在言语(MD,2.27;95%CI,1.89-2.65 vs 1.58;95%CI,1.00-2.16)和空间(MD,2.95;95%CI,2.66-3.24 vs 1.68;95%CI,0.96-2.39)听力质量上有较大改善。另外,设备非使用者的耳聋持续时间显著长于普通设备使用者的耳聋持续时间(中位数差异,6.84;95%CI,4.02-9.58)。

最后,研究人员指出,他们的系统性回顾和荟萃分析发现,SSD儿童的人工耳蜗植入与听觉和患者报告的结果有临床意义的改善相关;较短的耳聋持续时间可能会带来更好的植入结果。他们的这些发现可以指导未来的研究工作,完善人工耳蜗植入候选标准,并能够协助家庭咨询和共同决策。
