Eur Respir J:二氧化氮增加特发性肺纤维化患者死亡风险

增加接触NO2可能会增加IPF患者(尤其是老年男性)的死亡风险。

空气污染与特发性肺纤维化(IPF)患者的预后有关。近日,呼吸疾病领域权威杂志Eur Respir J上发表了一篇研究文章,研究人员旨在明确暴露于空气动力学直径≤10μm(PM10)和二氧化氮(NO2)颗粒物对IPF患者死亡率的影响。

该研究纳入了1995年至2016年间共诊断为IPF的1114例患者(平均年龄为65.7岁;男性占80.5%)。研究人员使用全国规模的暴露预测模型估算了患者居住地PM10和NO2水平的长期浓度。研究人员使用针对个体和区域水平协变量调整的Cox比例风险模型估算了PM10和NO2对死亡率的影响。

中位随访期为3.8年,所有患者中有69.5%的患者死亡或接受了肺移植。当对个体和区域水平的协变量进行调整后,NO2浓度增加10-ppb会导致死亡率增加17%(风险比[HR]为1.172 [95%CI:1.030-1.344,p=0.016])。当IPF患者按年龄(≥65岁 vs. <65岁)或性别进行分层时,NO2是老年人死亡的重要预后因素(HR为1.331 [95%CI:1.010-1.598,p=0.010])。如果按年龄和性别进行分层,则NO2与老年男性的死亡率相关性更强(HR为1.305 [95%CI:1.072-1.598,p=0.008])。在所有患者以及按年龄或性别分层的亚组中,PM10与IPF患者死​​亡率均无相关性。

由此可见,增加接触NO2可能会增加IPF患者(尤其是老年男性)的死亡风险。
