ANN ONCOL:动态分子分析与转移性结直肠癌临床进展之间的关系

作者收集了接受帕尼单抗联合伊立替康治疗的转移性结直肠癌患者的治疗前和治疗后肿瘤活检标本以及治疗前、治疗过程中和治疗后的血液样本,利用二代测序和BEAMing检测治疗过程中和治疗后的突变状况。二代测序发现的肿瘤组织RAS突变率为9.5%,BEAMing为6.3%。通过BEAMing检测血浆中的游离DNA发现RAS突变率为36.7%。对血浆样本的探索性分析表明在进展过程中出现RAS突变的患者与进展时保持野生型的患者相比中位无进展生存期相似。对血浆样本的连续分析结果表明进展前检测到出现RAS突变的中位时间为3.6个月,且未发现存在可以预测近期疗效的RAS突变频率阈值。

文章最后认为,该研究结果表明连续的血液活检在评估肿瘤异质性时所包含的信息比肿瘤组织活检更多。需进一步研究转移性结直肠癌患者血液检测的临床应用价值。
