European Radiology:CTP在缺血性脑卒中区域分类中的价值

对于急性缺血性卒中患者来说,CTP扫描可显示出不可逆转的受损组织(即梗死核心)和可挽救的组织(即缺血半暗带),其共同构成了总的低灌注区域。

随着血管内治疗越来越多的应用于急性缺血性卒中患者的治疗,基线成像对于选择治疗的患者来说变得更加重要其中计算机断层扫描灌注(CTP)成像可为临床提供个性化的选择标准。

对于急性缺血性卒中患者来说,CTP扫描可显示出不可逆转的受损组织(即梗死核心)和可挽救的组织(即缺血半暗带),其共同构成了总的低灌注区域。为了对这些区域进行分类,采用专用的软件进行CTP扫描图像的处理并产生四种灌注图:脑血流(CBF)、脑血量(CBV)、平均通过时间(MTT)和达峰时间(TTP/Tmax)。随后,应用预定的阈值来勾勒出缺血核心和缺血半暗带。

有学者提出可使用缺血性卒中区域的概率分类作为基于阈值分类的替代方法。概率图可以结合参数,并能表明缺血的确定性。然而对于在任何单一CTP软件中都可轻易获得的结合多个灌注参数的概率图来说,尚未有研究将概率分类与基于阈值的分类进行比较。

近日,发表在European Radiology杂志的一项研究使用商业方法(ISP)、bSVD和非线性回归(NLR)三种方法生成的CTP参数图比较了单一参数阈值与缺血性卒中区域的多变量概率分类,为患者的早期风险分层及治疗评估提供了技术支持。 

本研究共包括225个CTP数据集,分为166名成功再通的患者和59名持续闭塞的患者。CTP参数的精确性和召回率都低于合并成概率时。传统阈值法的估计梗死体积和随访梗死体积的中位数差异[四分位数范围]为29/23/23[52/50/52](ISP/bSVD/NLR),概率分类法为4/6/11[31/25/30](ISP/bSVD/NLR)。

本研究表明,多变量概率图在评估脑梗死病变方面优于传统的CTP摘要图,这一点是在3天内获得的随访平扫CT图像上观察到的。在临床上,改进的分类有利于对急性缺血性卒中患者进行不同治疗方案制定的筛选,概率图可以为CTP卒中成像中的缺血区提供一个改进的、标准化的分类手段。
