早发vs晚发的重症肌无力,在基因组学上有何差异?

重症肌无力是一种自身免疫性疾病,临床上表现为肌肉无力和易疲劳。大约80%的重症肌无力患者都有针对突触后肌肉内板的乙酰胆碱受体(AChR)的自身抗体。有趣的是,这些有AChR抗体的患者在发病时有一个双峰模式,早期高峰在30岁左右,晚期高峰在70岁左右。

早发性(EO)和晚发性(LO)的重症肌无力具有明显的流行病学和病理学特征。大多数EO患者有以生发中心为特征的胸腺增生,而这些特征在LO患者中不常见。此外,胸腺瘤相关的重症肌无力是另一个独特的群体,这些患者几乎都有可检测到的AChR抗体和全身性疾病。这些观察结果背后的机制仍未解决,但不同的免疫学途径已被提出来解释EO和LO形式的重症肌无力之间的差异。

最近的全基因组关联研究(GWAS)发现了一些重症肌无力的遗传风险位点,提供了对潜在疾病机制的一瞥。这些研究表明,不同的遗传风险因素可能有助于EO和LO重症肌无力的发病机制。值得注意的是,主要组织相容性复合体(MHC)位点内的不同遗传变异会导致EO和LO疾病。此外,通过GWAS也发现了MHC以外的几个风险位点;例如,TNIP1基因附近的遗传风险变异与EO疾病有关,而TNFRSF11A基因附近的变异则与LO疾病有关。尽管取得了初步成功,但重症肌无力症的GWAS相对来说力量不足,而且大多数风险变体都属于非编码区。因此,重症肌无力的大部分遗传易感性仍有待阐明。

值得注意的是,通过GWAS确定的每个遗传风险位点都包含许多与疾病相关的合理基因,需要进一步的工作来确定对因果连锁反应有贡献的基因。新出现的证据表明,因果遗传变异很可能通过基因表达的变化而产生功能效应,而且往往是以细胞类型的方式。此外,这些因果变体可能通过染色质循环调节远处基因的表达。

最近牛津大学的 Lahiru Handunnetthi等人,开发了Priority Index,这是一种基因组学方法,将细胞类型特异的功能基因组信息,如基因表达和染色质组织,与GWAS的发现相结合,以确定疾病相关基因。在这项研究中,他们在以前的工作基础上,将Priority Index应用于重症肌无力的GWAS结果,以确定疾病相关基因,并具体探索EO和LO重症肌无力之间的差异。


他们使用一种新的基因组方法,将重症肌无力症的基因组广泛关联研究(GWAS)结果与细胞类型的特定信息(如基因表达模式和启动子-增强子相互作用)相结合,以确定疾病相关的基因。随后,进行了额外的基因组调查,包括对313名健康人的表达定量分析,以提供机制上的见解。

根据GWAS相关的风险变异,他们发现了与早发性肌无力症特别相关的基因,包括TNIP1、ORMDL3、GSDMB和TRAF3。

还发现,toll样受体4信号的调节器仅在早发疾病基因中富集(富集倍数(fold enrichment)=3.85,p=6.4 x 10-3)。相比之下,T细胞调节器CD28和CTLA4只与晚期发病的疾病有关。

在CTLA4基因附近发现了两个因果遗传变异(rs231770和rs231735;后验概率= 0.98和0.91)。随后,我们证明这些因果变异导致CTLA4的低表达(rs231735和rs231770分别为rho=-0.66,p=1.28 x 10-38和rho=-0.52,p=7.01 x 10-22)。
