黑色素瘤和上皮癌通常在形成远处转移之前在淋巴结中形成转移。对人类和小鼠癌症(包括黑色素瘤)的遗传研究表明,局部淋巴结转移有时会引起远处转移。与此相一致,淋巴结中的癌细胞可以迁移到血管中,然后通过血液转移。但是,一些远处转移是由与采样淋巴结中不同的克隆引起的。在这些情况下,尚不清楚远处转移的细胞是直接从原发性肿瘤进入血液还是在进入血液之前先通过淋巴系统迁移。

在皮肤黑色素瘤患者中,局部淋巴结转移是远处转移和死亡的最重要预测指标之一。完全淋巴结清扫不能提高生存率,然而,这不一定意味着远处转移与淋巴系统无关,因为来自淋巴结的黑色素瘤细胞可能经常进入血液并在检测到局部转移之前扩散。

越来越多的证据表明,淋巴系统促进癌细胞的迁移和存活。与静脉注射后相比,一些癌细胞在注射入淋巴结后(鼻内注射)形成更多的肿瘤。血管内皮生长因子C(VEGF-C)和各种趋化因子促进癌细胞向淋巴管的迁移。代谢脂肪酸促进了癌细胞在淋巴系统中的存活和转移性肿瘤的形成。然而,循环中的癌细胞主要是在血液中表征的,关于这些细胞与淋巴管中的癌细胞相比的信息很少。尽管一些癌细胞在进入血液并形成远处转移之前就已通过淋巴管迁移,但接触淋巴液是否会影响这些细胞在血液中的后续存活尚不清楚。

通过血液转移是一种效率很低的过程,几乎没有癌细胞能够存活。限制生存的一个因素是氧化应激。然而,尚不清楚氧化应激如何杀死转移细胞。脂质氧化可引起铁死亡-一种细胞死亡的模式,其中磷脂中的多不饱和脂肪酸被氧化还原活性铁氧化。尽管癌细胞可以在体内发生铁死亡,但大多数铁死亡研究都是在培养物中进行的,这限制了我们对铁死亡影响体内癌症生长或进展的情况的理解。
