NEJM:肺癌治疗进展对死亡率的影响

2013-2016年,美国非小细胞肺癌患者的死亡率大幅下降,存活率大幅提高。随着治疗进展,发病率的降低,特别是靶向治疗的广泛使用是非小细胞肺癌患者死亡率下降的重要因素

肺癌,包括非小细胞肺癌(NSCLC)和小细胞肺癌(SCLC),在美国肺癌的总死亡率呈现出下降的趋势,但肺癌亚型对死亡率趋势的影响研究尚不充分。
 
SEER研究中,研究人员评估了美国特定肺癌患者死亡率变化趋势,还根据肺癌亚型分析肺癌的发病率和存活率,通过Joinpoint系统评估不同类型肺癌发病率的变化和基于发病率的死亡率变化趋势。
 
NSCLC患者的死亡率呈快速下降的趋势,其速度超过发病率的下降程度,NSCLC患者死亡率下降与生存率的显著提高相关,其中靶向治疗是决定性因素。在男性群体中,2013-16年间,NSCLC的死亡率每年下降6.3%,而2008-16年间,发病率每年下降3.1%。NSCLC男性患者的特异性生存率从2001年的26%,提高到2014年的35%。在所有种族和族裔群体中NSCLC的死亡率均显著下降。女性NSCLC患者死亡率下降趋势与男性相同。与NSCLC不同,
非小细胞肺癌死亡率、发病率与生存率变化
SCLC患者死亡率的下降幅度与发病率下降幅度相当,患者生存率没有显著改善,其中SCLC治疗进展有限是最主要的因素。
