Eur Respir J:职业性粉尘暴露与纵向肺功能下降无关

近年来职业暴露于矿物粉尘、生物粉尘、气体与肺功能加速下降无关。

有研究证明,职业性空气暴露与肺功能下降和COPD有关。Eur Respir J杂志的一项研究探索了职业空气暴露与肺功能变化之间的关系。

研究数据来自两项一般人群的队列研究,哥本哈根城市心脏研究(CCHS)和哥本哈根一般人口研究(CGPS),平均随访9年(范围3-27年),至少进行两次肺功能测量。考虑以下主要类别的空气传播媒介:矿物粉尘、生物粉尘、气体和烟雾,以及复合气体、灰尘或烟雾。根据队列和时间段(1976-1983年和2003-2015年),使用线性混合效应模型研究每年平均职业暴露与FEV1平均年下降之间的相关性。随访期间校正了性别、身高、体重、教育程度、基线FEV1和每年吸烟包年。

研究共纳入16144人(平均年龄48岁,男性占43%),吸烟者比例分别为61%和20%。563人完成三次肺功能测量。暴露分析中,矿物粉尘、生物粉尘、气体和烟雾以及复合类别的职业暴露与FEV1下降无关。每天20支香烟与FEV1变化相关,FEV1变化为-17mL/年(95% CI, -19~ -15)。此外,基线FEV1和女性与FEV1的变化显著相关。暴露指数测量分析中,1976-1983年期间气体和烟雾与FEV1变化-5.8 mL/单位/年(95%CI, -10.8 ~ -2.3)相关,但在2001-2015年期间无关。

结果表明,来自丹麦普通人群的两个队列中,近年来职业暴露于矿物粉尘、生物粉尘、气体与肺功能加速下降无关,但可能在几十年前具有重要意义。
