新冠肺炎病毒肆虐全球,此起彼伏,秋冬将至,防止疫情野火复燃二次爆发的任务也越来越紧迫。该病毒感染不仅可导致重度肺炎(新冠肺炎[COVID-19])和呼吸衰竭,还可累计肾脏和心肌,但迄今为止,导致呼吸衰

新冠肺炎病毒肆虐全球,此起彼伏,秋冬将至,防止疫情野火复燃二次爆发的任务也越来越紧迫。该病毒感染不仅可导致重度肺炎(新冠肺炎[COVID-19])和呼吸衰竭,还可累计肾脏和心肌,但迄今为止,导致呼吸衰竭和心肾受累的机制仍未完全明确。一波不平,一波又起,COVID-19患者出现全身性血栓形成表型的报道也屡见不鲜。

本研究纳入了62位个体,包括38位通过qPCR确诊的COVID-19患者和24位非COVID-19对照。研究人员对尸检病例进行了组织病理学评估,对中性粒细胞和血小板进行了基于表面标志物的表型分析,并进行了血小板、中性粒细胞功能和凝血功能测定。

研究结果表明,COVID-19患者的器官受累和血栓形成特征与免疫血栓形成有关。在COVID-19患者中,炎性微血管血栓存在于肺、肾和心脏中,含有与血小板和纤维蛋白相关的嗜中性粒细胞外陷阱。

COVID-19患者还表现出中性粒细胞-血小板聚集,以及血液中明显的中性粒细胞和血小板激活模式,且随疾病的严重程度而变化。与健康对照和非COVID-19肺炎个体相比,中度COVID-19肺炎患者表现出血小板耗竭和中性粒细胞低激活表型,而重度患者则表型为血小板和中性粒细胞过度激活。重度新冠肺炎患者的免疫血栓调节异常与急性呼吸窘迫综合症和全身性高凝状态均相关。

总之,该研究表明,免疫血栓失调是COVID-19疾病严重程度的关键标志。为了确定免疫血栓形成在COVID-19中的作用,还需要进一步研究。
