Clin Chem:补充一年维生素D和海洋Omega-3脂肪酸都不会降低老年人的系统性炎症状态

本项随机、双盲、安慰剂对照的VITamin D和OmegA-3 试验(VITAL)测试了维生素D(2000 IU/天)和/或n-3 FA(1 g/天)补充剂,在年龄≥55岁的女性和≥50岁的男性中进行2×2因子设计。评估了随机接受维生素D+n-3 FA(392)、维生素D(392)、n-3 FA(392)或仅安慰剂(385)的参与者中,白细胞介素(IL)-6、肿瘤坏死因子受体2(TNFR2)和高敏C反应蛋白(hsCRP)浓度从基线到1年的变化。比较了几何平均数和百分比变化,校正了基线因素。

结果显示,基线特征非常匹配。试验组中,25-OH维生素D的水平上升了39%,n-3 FA的水平上升了55%,而安慰剂组的变化很小。两种补充剂都没有降低1年时的生物标志物。维生素D导致IL-6的水平升高8.2%(95%CI,1.5%-15.3%;校正后P=0.02),但TNFR2和hsCRP没有。在784名接受维生素D的患者中,hsCRP的水平增加35.7%(7.8%-70.9%),但基线血清25(OH)维生素D水平较高的患者没有增加[0.45%(-8.9%~10.8%);P交互作用=0.02]。在777名随机使用n-3 FA的人中,基线低(<1.5份/周)的人hsCRP下降[-10.5%(-20.4%至0.8%)],但鱼类摄入量较高的人没有下降[6.4%(95% CI,-7.11%至21.8%);P交互作用=0.06]。

 

综上所述,该研究结果表明,在这个基于人群的随机对照试验的大样本中,1年内补充维生素D或n-3 FA都不会降低这些炎症的生物标志物。
