DCR: 肌肉和脂肪组织大肠癌患者长期生存的影响

背景:
计算机断层扫描术得出的身体成分参数是直肠癌的新兴预后指标。这项研究的目的是确定肌肉减少症和肥胖作为I至III期大肠癌的独立预测作用。

方法:
这是一项来自前瞻性收集数据库的回顾性队列研究。研究人员进行多元Cox比例风险模型评估身体组成参数与生存之间的联系。研究人员将2007年至2009年接受根治性切除的I至III期大肠癌患者纳入本项研究。使用计算机断层扫描法得出的骨骼肌和脂肪组织的定量结果可用于确定少肌症,肌脂减少和总肥胖的人群特异性临界值。主要观察结局指标为总体,无复发和癌症特异性生存率。

结果:
在968例患者中,共有254例疾病复发和350例死亡。体重指数和CT得出的肥胖测量结果并未导致较差的生存结果。肌肉减少症可独立预测总体恶化(HR,1.45; 95%CI,1.16-1.84),无复发(HR,1.32; 95%CI,1.00-1.75)和癌症特异性生存率(HR,1.46; 95% CI,1.09–1.94)。肌脂肪变性也可以独立预测总体存活率(HR,1.53; 95%CI,1.19-1.97)。

结论:
肌肉减少症和肌脂肪变性是I至III期大肠癌生存率降低的独立预测因子。
