目前,关于低血浆25-羟维生素D浓度是否会导致骨质疏松性骨折尚不清楚。本研究使用孟德尔随机分析对血浆25-羟基维生素D浓度低与骨质疏松性骨折风险增加相关的假设进行了验证。

研究人员随机选择了116 335名20-100岁的丹麦白人进行2项基于人群的队列研究,对CYP2R1 (rs117913124和rs12794714)、DHCR7 (rs7944926和rs11234027)、GEMIN2 (rs2277458)和HAL (rs3819817)基因型进行了研究,35833例患者包含血浆25-羟基维生素d的信息。研究人员评估了1981年至2017年期间发生全骨、骨质疏松和局部解剖性骨折的风险。从全国注册中心获取有关骨折和生命状态的信息。

在长达36年的随访中,研究人员观察到17820例骨折,10861例骨质疏松性骨折,3472例髋或股骨骨折。与25-羟基维生素D≥50nmol/L的个体相比,与25-羟基维生素D≥50nmol / L的个体相比,对于25–49.9 nmol / L的个体,总骨折的多变量调整后危险比(95%CI)为1.03(0.97–1.09),25-羟基维生素D <12.5 nmol / L的人为1.39(1.21-1.60),12.5-24.9nmol / L为1.19(1.10–1.28)。 骨质疏松性骨折和髋部或股骨骨折的相应危险比分别为1.07(1.00-1.15),1.25(1.13-1.37)和1.49(1.25-1.77),以及1.09(0.98-1.22),1.37(1.18-1.57)和1.41(1.09-1.81)。维生素D等位基因评分每增加1个风险比,即25-羟基维生素D浓度降低3.0%(约1.6 nmol/L),全骨折的风险比为0.99(0.98 - 1.00),骨质疏松性骨折的风险比为0.99(0.97-1.00),髋或股骨骨折的风险比为0.98(0.95-1.00)。

研究结果表明,低血浆25-羟基维生素D浓度与骨质疏松性骨折相关;然而,孟德尔随机化分析并没有提供证据支持维生素D在骨质疏松性骨折风险中的因果作用。
