J Diabetes:实锤!睡眠效率低将大大增加糖尿病风险!

随着糖尿病相关研究的不断深入,研究者发现睡眠与糖尿病也存在千丝万缕的联系,如失眠、睡眠呼吸暂停等睡眠障碍会增加糖尿病发生风险。近期,发表在Journal of Diabetes上一项研究显示无睡眠呼吸紊乱的成年人出现低效率睡眠可能会增加其罹患糖尿病的风险。

随着糖尿病相关研究的不断深入,研究者发现睡眠与糖尿病也存在千丝万缕的联系,如失眠、睡眠呼吸暂停等睡眠障碍会增加糖尿病发生风险。近期,发表在Journal of Diabetes上一项研究显示无睡眠呼吸紊乱的成年人出现低效率睡眠可能会增加其罹患糖尿病的风险。

研究概况

西安交通大学医学院颜斌博士等人分析了1995~1998年参与睡眠心脏健康研究的4737例成人的睡眠相关数据及糖尿病患病情况。睡眠相关数据来源于家庭多导睡眠图监测的睡眠特征客观指标,包括睡眠效率、睡眠潜伏期、慢波睡眠、睡眠后觉醒、总觉醒指数和呼吸暂停低通气指数,其中,睡眠效率通过床上时间/实际睡眠时间表示,分为1级(>85%)、2级(80%~84.9%)和3级(<80%);依据呼吸暂停低通气指数的数据,以每小时至少有5次呼吸暂停低通气事件为睡眠呼吸紊乱的阈值。

主要结果

结果显示在4737例受试者(平均年龄63.6±11.0岁)中,伴有睡眠呼吸紊乱的受试者相比无睡眠呼吸紊乱的受试者患有糖尿病比例更高(9.3% vs 5.3%; P<0.001)。在整个研究队列中,3级睡眠效率组的糖尿病比例高于1级和2级睡眠效率组(10.3% vs 7.1% vs 5.7%;P<0.001)。

进一步分析发现,在无睡眠呼吸障碍的人群中,3级睡眠效率组的糖尿病患病率高于1级和2级睡眠效率组(分别为9.5% vs 5.6% vs 3.5%;P<0.001);在患有睡眠呼吸障碍的人群中,3级睡眠效率组的糖尿病患病率高于1级和2级睡眠效率组,但差异无统计学意义(分别为10.9% vs 8.5% vs 8.3%;P=0.134)。

在调整潜在的混杂因素后,仅在无睡眠呼吸紊乱人群中,睡眠效率<80%与糖尿病患病率升高相关(OR=1.894;95%CI:1.187~3.022,P=0.007)。因此,无睡眠呼吸紊乱人群的睡眠低效率可能会增加糖尿病发生风险。

研究者评述

该研究的主要研究者指出,“此项研究结果显示无睡眠呼吸紊乱人群的睡眠效率低下与糖尿病发生风险增高有关,但睡眠呼吸紊乱人群的睡眠效率低下与糖尿病风险无关。睡眠效率与糖尿病之间的因果关系仍需要进一步探讨。睡眠效率低的人更容易睡眠质量差,这可能会改变食欲相关激素瘦素的水平。而瘦素可以与大脑中相应的受体结合,控制进食行为、能量消耗和葡萄糖调节。此外,睡眠效率低下可能导致糖耐量降低、胰岛素敏感性降低和急性胰岛素抵抗减少。睡眠效率可能导致糖尿病发生风险的机制还需要进一步探索”。
