Nat Commun:老年人骨质疏松症却容易血管钙化的“钙化矛盾”得到破解

老年人往往有骨质疏松,但是又同时会伴有血管钙化,这种钙的不平衡矛盾,一直难以得到合理解释。甚至产生,老年人该不该补钙的矛盾。以前的观点认为,补钙可能导致血管钙化的发生,对心血管有一定的风险性。但现在的

老年人往往有骨质疏松,但是又同时会伴有血管钙化,这种钙的不平衡矛盾,一直难以得到合理解释。甚至产生,老年人该不该补钙的矛盾。以前的观点认为,补钙可能导致血管钙化的发生,对心血管有一定的风险性。但现在的学术共识,认为大剂量补钙才可能导致血管钙化。按照美国国家骨质疏松基金会和美国预防心脏病协会的建议,只要每日摄入钙的总量不超过2000mg~2500mg,就认为对心血管系统是安全的。另外,虽然血管钙化主要是在血管粥样硬化的基础形成,但医学界对血管钙化的看法,并不是更严重的血管硬化,相反,有理论认为血管钙化有可能增加粥样硬化斑块的稳定性,对血管会产生一定的保护作用。但这些都没有具体的机制。

近日,中南大学湘雅医院谢辉教授团队在骨质疏松症与血管钙化的发生发展中衰老骨基质来源的细胞外囊泡的作用及机制方面取得进展。研究成果以“衰老骨基质来源胞外囊泡作为信使参与钙化矛盾(Aged bone matrix-derived extracellular vesicles as a messenger for calcification paradox)”为题,于2022年3月18日在线发表于《自然×通讯》(Nature Communications)。

骨质疏松与血管钙化是严重威胁中老年人健康的两种病理现象。随着机体的不断衰老,骨骼中的钙丢失增加、骨量减少,逐渐形成骨质疏松,而血管壁组织中的钙沉积增多,导致血管的僵硬性增加、顺应性降低,从而在骨骼与血管壁组织中产生了“钙化矛盾”。临床和基础研究表明这两种病理现象与年龄存在密切联系。我国的人口老龄化正在不断加剧,阐明骨质疏松与血管钙化的共同发生机制,具有重要的科学意义和临床应用前景。

该团队研究发现,骨细胞可分泌骨基质细胞外囊泡(B-EV),在骨吸收的过程中B-EV被释放至骨髓腔中,通过血液循环转运到血管壁组织;来源于老年人群(67—73岁)的B-EV能显著促进骨髓间充质干细胞的成脂分化和血管平滑肌细胞的成骨转分化;将来源于老年人群的B-EV经骨髓腔注射到年轻或老年小鼠体内,能导致小鼠的骨量减少和骨髓腔脂肪增多;将其经尾静脉或骨髓腔注射至血管钙化小鼠模型体内,能显著加剧血管钙化程度;来源于年轻人群(27—31岁)的B-EV则无上述作用。

该团队还发现骨吸收抑制剂阿仑膦酸钠能显著抑制衰老机体B-EV的释放,并缓解老年去卵巢小鼠的骨质疏松与血管钙化。进一步研究发现,衰老机体来源的B-EV分别通过富集miR-483-5p和miR-2861来促进骨髓间充质干细胞成脂分化与诱导血管平滑肌细胞成骨转分化(图)。

该团队对B-EV在骨代谢和血管钙化中的作用及机制进行了深入的研究,为老年骨质疏松症和血管钙化的防治提供了新思路。
