Circulation:重度二尖瓣反流继发心肌细胞肥大的分子机制

原发性瓣膜性心脏病是工业化国家和发展中国家发病率和死亡率的普遍原因。虽然瓣膜性心脏病的主要后果是心肌功能障碍,但对瓣膜性心脏病的治疗主要围绕瓣膜修复或替换,而不是预防或逆转心肌功能障碍。这在原发性二尖瓣返流(MR)中尤为明显,如果不及时进行瓣膜修复或更换,MR必然会导致偏心肥厚和左心室(LV)衰竭。但是,迄今为止,原发性重度MR左室功能障碍的机制尚不清楚。

Li Shujuan等人建立了第一个重度MR的小鼠模型,是通过切断二尖瓣瓣叶诱导而来,并通过超声心动图证实。采用连续超声心动图随访左室形态和收缩功能。随后心脏组织分析评估瓣膜变形、心肌细胞形态、左室纤维化和细胞死亡。最后,通过RNA测序和免疫组化分析失调的信号通路。

在成功诱导MR后的15周内,左室逐渐扩张,并出现功能障碍,导致严重的收缩功能障碍。进一步分析显示重度MR导致心脏重量显著增加,心肌细胞长度也增加,但心肌细胞宽度未发生改变;电子显微镜下可见肌节紊乱和肌节破坏。

机制上,重度MR导致哺乳动物雷帕霉素靶蛋白和钙调神经磷酸酶通路的多个成分激活。抑制哺乳动物雷帕霉素靶点可保护肌节结构,预防LV重构和收缩功能障碍。免疫组化分析发现细胞极性调节因子Crb2在MR心脏中沿心肌细胞纵轴和靠近润盘位置的表达模式存在差异。电子显微镜图像显示在MR心脏中,多聚体定位明显增加,靠近润盘和一些沿纵轴的区域。

这些结果表明,重度MR引起的左室功能障碍是心肌细胞适应性偏心性肥大的一种形式,并概述了细胞极性调节和空间定位蛋白合成之间的联系,这是心肌细胞定向生长的途径。
