研究发现米非司酮预处理后使用米索前列醇可提高早早孕丢失患者首次妊娠囊排出率

早早孕丢失，即临床未能识别的自然流产，可选择吸宫术或米索前列醇药物治疗，但药物治疗失败率较高。近日研究人员考察了米非司酮联合米索前列醇治疗早早孕丢失的效果。

300名无胚妊娠、胚胎或胎儿死亡女性参与研究，接受200 mg米非司酮，之后接受800μg米索前列醇或仅接受800μg米索前列醇治疗，患者服药后1-4天接受超声随访，未排出妊娠囊女性接受二次米索前列醇或子宫抽吸。研究的主要终点是首次治疗后妊娠囊排出率以及30天无额外治疗干预率。

米非司酮组124名参与者（83.8%）以及米索前列醇组100名参与者(67.1%)在首次服药后妊娠囊完全排出，米非司酮组参与者需子宫吸引的患者比例显著降低（8.8% vs 23.5%）。米非司酮组因输血导致的出血率为2.0%，而米索前列醇组为0.7%，各组骨盆腔炎发生率均为1.3%。

研究发现米非司酮预处理后使用米索前列醇可提高早早孕丢失患者首次妊娠囊排出率。
