JCEM:单核细胞计数减少与妊娠糖尿病、巨大儿和炎症相关

整个妊娠期间单核细胞计数减少与GDM、巨大儿和GDM的慢性炎症状态的发展密切相关。孕早期单核细胞计数作为GDM的早期诊断标志物具有巨大潜力。

免疫系统在妊娠期糖尿病(GDM)的病理生理学机制中起着核心作用。单核细胞是主要的先天免疫细胞,对于维持正常妊娠尤为重要。

近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,研究人员分析了单核细胞在GDM中的潜在效应。

在病例对照和队列研究中,研究人员监测了214名患有GDM的女性和926名没有GDM的女性整个妊娠期间的单核细胞计数,并测量了循环中炎性细胞因子、胎盘来源的巨噬细胞及其产物的水平。


在整个妊娠期间,GDM女性的单核细胞计数显著降低,并且与血糖水平、胰岛素抵抗和新生儿体重密切相关。在病例对照和队列研究中,作为GDM和巨大儿的危险因素和诊断预测因子,早孕期单核细胞计数优于中孕期和孕晚期单核细胞计数。此外,该队列研究表明,随着妊娠早期单核细胞计数的减少,GDM和巨大儿的发生率、血糖水平和新生儿体重呈阶梯式增加。当妊娠早期单核细胞计数超过0.48×109/L时,GDM的风险开始迅速降低。值得注意的是,CD206和IL-10显著降低,而CD80、CD86、TNF-α和IL-6在GDM胎盘组织和外周血中均较高。孕早期单核细胞计数与IL-10和CD206呈正相关,但与CD80、CD86、TNF-α和IL-6呈负相关。
 
由此可见,整个妊娠期间单核细胞计数减少与GDM、巨大儿和GDM的慢性炎症状态的发展密切相关。孕早期单核细胞计数作为GDM的早期诊断标志物具有巨大潜力。
