目前诸多研究表明低度的炎症反应参与了2型糖尿病和心血管疾病(CVD)的发生发展,但评估炎性标记物预测胰岛素分泌和胰岛素敏感性变化的前瞻性研究尚缺乏。

2017年7月在JCEM上发表的一篇文章研究了男性代谢综合征(METSIM)前瞻性研究中糖蛋白乙酰(GlycA)、白细胞介素-1受体拮抗剂(IL-1RA)和高敏C反应蛋白(hs-CRP)与胰岛素分泌、胰岛素敏感性、2型糖尿病高血压、CVD事件和总体死亡率。该横断面研究纳入了8746例从芬兰库奥皮奥人口登记中随机抽取的45到73岁的非糖尿病男性,其中5401人参加了6.8年的随访。

在随访期间,GlycA与胰岛素分泌受损、高血糖、2型糖尿病的发生[危险比(95%可信区间):1.37(1.26-1.46)]和CVD[1.21(1.12-1.32)]有关。IL-1RA和hs-CRP与胰岛素敏感性、肥胖相关特性的不良变化、总死亡率 [ 分别为1.13(1.07-1.20)和1.08(1.04-1.11)]有关。

炎性标志物不同地预测了胰岛素分泌和胰岛素敏感性的变化。GlycA预计受损的胰岛素分析,而IL-1RA和hs-CRP预测了胰岛素敏感性的变化。结合这3个标记物改善了对疾病预测的结果,也表明他们捕捉到低度炎性反应的不同方面。
