本研究旨在评估滑膜α-defensin酶联免疫吸附试验(ELISA)在全髋关节(THA)和膝关节置换术(TKA)术前的工作检查中,评估滑膜α-defensin酶联免疫吸附试验(ELISA)对人工关节感染(PJI)诊断的准确性。

 

本前瞻性队列研究的纳入标准为指数关节的急性或慢性症状,无特定的排除标准。对202例患者的滑膜液抽吸液进行了分析,并进行了半定量实验室α-defensin ELISA检测。通过对复诊手术中获得的样本进行检查,最终确定了PJI的诊断。

 

结果显示,α-defensin ELISA对PJI的敏感性和特异性分别为78.2%(95%置信区间(CI)66.7~88.5)和96.6%(95%CI 93.0~99.3)。阳性和阴性预测值分别为89.6%(95% CI 80.6至97.8)和92.2%(95% CI 87.5至96.1)。在22%的败血症复查中,该试验仍为假阴性,其中大部分是由于凝血酶阴性葡萄球菌全部发生在术后晚期或术后早期PJI中。

 

总之,该研究结果表明,在术前评估无症状的THA和TKA时,常规使用滑膜液α-defensin实验室ELISA法,不足以准确诊断PJI。特别是在涉及低致病菌(如凝血酶阴性葡萄球菌)的病例中,仍然需要更高的检测灵敏度。
