Lancet子刊:间充质干细胞治疗多发性硬化症?可能效果并没有想象的好!

这项研究不支持使用骨髓来源的间充质干细胞来治疗活动性多发性硬化症。

此前的研究表明,间充质干细胞(MSCs),也被称为间充质基质细胞,基于其免疫调节和神经保护特性,已被提议作为多发性硬化症患者的一个有希望的治疗选择。近期,来自意大利热那亚大学神经科学系的专家开展了MESEMS研究,旨在评估来自骨髓的自体间充质干细胞用于治疗多发性硬化的安全性、耐受性和活性。结果发表在Lancet Neurology杂志上。

MESEMS是一项在9个国家的15个地点进行的随机2期试验。患有活动性复发缓解型或进行性多发性硬化症的患者(18-50岁),纳入病程为2-15年,并且扩展的残疾状态量表评分为2.5-6.5。

根据交叉设计,患者被随机分配(1:1)接受单次静脉注射自体骨髓间充质干细胞,然后在第24周接受安慰剂,或在第24周接受安慰剂,然后接受自体间充质干细胞,并在第48周进行随访。主要目标是测试MSC治疗的安全性和活性。主要安全终点是评估每个治疗组内不良事件的数量和严重程度。

主要疗效终点是比较各治疗组在第4、12和24周的钆增强病变(GELs)的数量。主要疗效终点是在所有参与者完成第24周的访问后,在完整的分析集中进行评估。疗效终点采用预定的统计测试程序进行评估。在整个研究过程中,通过记录每次访问的生命体征和不良事件来监测安全性。

从2012年7月16日-2019年7月31日,144名患者被随机分配到先接受自体骨髓间充质干细胞(n=69)或安慰剂(n=75)的早期静脉注射。间充质干细胞治疗在从基线到第24周积累的GELs总数上没有达到主要疗效终点(比率[RR]0.94,95%CI 0.58-1.50;P=0.78)。

研究期间记录了213例不良事件,组间分布相似(69名患者中的35名[51%]记录了93例,而75名患者中的42名[56%]记录了120例,先输注安慰剂)。报告的最常见的不良事件是感染,213例不良事件中共有54例(25%)(早期MSC组93例中的18例[19%]和延迟MSC组120例中的36例[30%])。

综上,骨髓来源的间充质干细胞治疗是安全和耐受性良好的,但在第24周时,对活跃形式的多发性硬化症患者的GELs没有显示出影响。因此,这项研究不支持使用骨髓来源的间充质干细胞来治疗活动性多发性硬化症。进一步的研究应解决MSCs对组织修复相关参数的影响。

干燥综合征(SS)是一种慢性的自身免疫性疾病,通常会损害外分泌腺(主要是唾液腺和泪腺)的正常功能,导致口干和眼睛干燥等症状。免疫系统的功能异常已被证明在SS的发病机制中起着至关重要的作用。

Richard Burt等人研究并建立了成人急性淋巴细胞白血病(ALL)的间充质基质细胞(MSC)生态位模型。研究人员通过基因表达谱、细胞因子/趋化因子定量、流式细胞术和多种成像技术证实,MSC是直接来自于ALL患者的原始骨髓样本,这些MSC都具有活化的、与癌症相关的成纤维细胞表型。

间变性大细胞淋巴瘤(ALCLs)是一组比较常见的T细胞非霍奇金淋巴瘤(T-NHLs),它们具有相似的病理特征,但具有显著的遗传异质性。根据ALK重排的存在与否,ALCLs大致可分为ALK阳性或ALK阴性。对62例T-NHLs患者的外显子测序发现在肌球蛋白基因上有一个既往未报道过的复发性突变,MSCE116K,仅见于ALK阴性的ALCLs。对238例T-NHLs患者进行额外测序证实了MSCE116K
