JAMA Neurology:高血压加速痴呆发生,那么强化降压能预防老年痴呆吗?

痴呆是当今全球重要健康挑战,迄今未有逆转或治愈痴呆的有效策略。去年6月,复旦大学附属华山医院神经内科郁金泰教授发表在最新一期高血压领域顶级期刊《高血压》(Hypertension)杂志上的研究成果表明,血压与痴呆神经病理变化等相关。

研究人员发现,中年高血压会显著增加认知障碍的发生风险。当中年高血压患者收缩压超过130mmHg水平时,认知障碍风险显著上升。同时,晚年高收缩压、低舒张压、血压调节异常等都会增加认知障碍的发生风险。

研究显示,晚年舒张压控制在90至100mmHg水平,最常见的痴呆——阿尔茨海默病(AD,老年痴呆)的发生风险最低。随后,有荟萃分析表明,改善高血压控制可以降低认知障碍和痴呆的风险。然而,目前尚不清楚反映AD病理的途径在多大程度上受到高血压控制的影响。

为此,美国宾夕法尼亚大学神经科学专家携手国际收缩压干预试验(SPRINT)研究组,全面评价了强化血压控制对AD相关脑部生物标志物的关联。结果发表在最新的《美国医学会杂志》子刊JAMA Neurology上。

SPRINT MIND是一项多中心随机临床试验,比较了常规降压与强化降压2种不同降压策略的疗效。对50岁以上高血压且无糖尿病或中风病史的参与者进行脑MRI研究(n = 1267)。最终有673名和454名参与者分别在基线和4年随访时完成了脑MRI。

参与者被随机分配到收缩压目标<120 mmHg(强化治疗组:n = 356)或<140 mmHg(标准治疗:n = 317)。研究的主要结果是观察海马体积的变化,AD患者大脑后扣带回脑血流和扣带束的平均变异系数。

结果显示,在673名招募的基线MRI患者中(平均67.3;271名女性[40.3%]),454名患者进行了为期3.98(3.7-4.1)年的随访,并完成相关MRI检查。总得来说,两组AD患者脑域萎缩、脑血流或平均分布异性参数没有显著差异。强化治疗组,平均海马体积从7.45 cm3减少到7.39 cm3,而标准治疗组则与从7.48 cm3减少到7.46 cm3。

海马萎缩是典型的轻度认知障碍和AD的最研究的标志之一,研究人员推测,强化血压治疗可能与这种萎缩的特定AD脑部测量有关,也与区域皮质厚度等其他因素有关。令人惊讶的是,与标准的降压治疗相比,强化降压可以延缓海马体积减小。

综上,强化高血压控制并不能减轻老年痴呆的风险,但可以延缓海马体积缩小。
