J Dent Res:将干细胞封存生物材料中可用于治疗根尖周病变再生牙髓

一项新的随机对照I/II期临床试验旨在评估将人脐带间充质干细胞封装在血浆衍生生物材料中,用于有根尖病变的成熟恒牙的牙髓再生治疗程序(REPs)的安全性和有效性。

一项新的随机对照I/II期临床试验旨在评估将人脐带间充质干细胞封装在血浆衍生生物材料中,用于有根尖病变的成熟恒牙的牙髓再生治疗程序(REPs)的安全性和有效性。

 

该试验纳入了36名患者伴有牙髓坏死和根尖牙周炎的切牙、尖牙或下颌前磨牙。将患者随机分配实验组(REP)或常规根管治疗组(ENDO)。在第一次就诊时,进行根管探查和机械备。使用氢氧化钙药物封药。3周后,患者按照指定的方案ENDO或REP进行治疗。6个月和12个月进行临床随访检查。采用费希尔精确检验进行评估分类变量。使用曼氏惠特尼检验进行比较定量变量。探讨随着时间推移,髓腔灌注单位的百分比病变和皮层损害大小变化

 

结果,在12个月的随访后,没有不良事件的报道,两组患者均显示出100%的临床疗效。有趣的是,在REP组中,通过激光多普勒血流仪测量显示从基线到12个月随访髓腔灌注单位百分比60.6%增加78.1%。敏感性试验显示,REP组的牙髓反应阳性率在12个月时有所增加(冷从6%上升到56%,热从0%上升到28%,电从17%上升到50%)。

 

总之,本研究中,我们首次提出了异源性脐带间充质干细胞封装在血浆衍生生物材料中用于牙髓治疗的临床安全性和有效性证据。该创新方法基于促进牙髓再生的生物学原理,为根尖周病变的治疗提供了一种有希望的选择
