心脏肥大是心脏疾病发展的关键阶段,其特征包括心肌细胞肥大和纤维化,这最终可能导致心力衰竭。REST(RE1沉默转录因子)在胚胎发育和胎儿心脏基因的重激活中扮演着重要角色,但其与心脏肥大的具体关系尚不明确。

上海同济大学的Luying Peng/Li Li教授团队在本期刊发表了一篇题为“The silencing transcription factor REST targets UCHL1 to regulate inflammatory response and fibrosis during cardiac hypertrophy”的研究快讯,该研究揭示了REST通过靶向UCHL1来调控心脏肥大过程中的炎症反应和纤维化,为心脏肥大的治疗提供了新的靶标。

1.   研究方法

研究团队构建了横向主动脉缩窄(TAC)的小鼠心脏肥大模型以及体外培养的心肌细胞模型。他们利用腺病毒载体来沉默REST的表达,并检测了REST缺失对心脏肥大、炎症反应和纤维化的影响。此外,他们还研究了REST对UCHL1表达的调控作用,并评估了UCHL1抑制剂对REST缺失型心脏肥大的影响。

2.   研究结果

在Ang II诱导的心肌肥大模型和TAC模型中,REST的mRNA和蛋白水平均显著下调(图1B、C)。在TAC处理后,REST基因敲除小鼠表现出更明显的心肌细胞肥大和胶原沉积(图1D–F)。REST的缺失促进TAC诱导的炎症因子IL-1β、IL-6和TNF-α的表达上调(图1G)。在TAC小鼠中,REST的缺失增加了IκBa和NF-κB(p65)的磷酸化水平(图1H),并激活JAK2和STAT3通路(图1I),而过表达REST则抑制了这些通路的激活。

REST的缺失还诱导UCHL1表达上调。在REST基因敲除小鼠中,使用UCHL1抑制剂LDN-57444处理可显著减轻心肌肥大、炎症和纤维化,并降低NF-κB和STAT3的磷酸化水平(图1J、K)。

3.   研究结论

本研究证实,REST在心肌肥大中起到保护作用,通过靶向UCHL1调节炎症反应和纤维化,这一机制涉及对NF-κB和JAK/STAT信号通路的调控。REST的缺失加剧了心肌肥大和纤维化,而UCHL1抑制剂LDN-57444的应用可减轻REST缺失引起的心肌肥大和炎症反应,为心肌肥大的治疗提供了新的潜在靶点和干预策略。
