脂肪酸氧化(FAO)增强一直被认为是肥胖/糖尿病诱导性心肌病发展的罪魁祸首。但是,通过敲除乙酰辅酶A羧化酶2(ACC2)消除长链脂肪酸转运到线粒体的抑制来增强心肌FAO并不能使非肥胖小鼠罹患心肌病,提示高FAO与心脏脂质毒性不同。研究人员推测,心脏病理学相关的肥胖归因于脂肪酸供应和氧化失衡。本研究旨在明确通过诱导ACC2缺失来进一步增强FAO水平是否可以预防肥胖引起的心肌病,同时阐明其潜在机制。

研究人员采用他莫昔芬诱导性模型,通过特异性敲除心肌细胞的ACC2来诱导高FAO水平。用高脂饮食(HFD)喂养对照和ACC2 iKO小鼠24周来诱导肥胖。

(敲除ACC2可减轻HFD诱导的心肌纤维化[红色染的纤维蛋白])

虽然对照和ACC2 iKO小鼠都表现出相似的肥胖表型,但通过敲除ACC2增强FAO氧化可预防HFD诱导性心肌功能障碍、病理性心室重构和线粒体功能障碍。与此类似,在体外,通过敲除ACC2增强FAO还可预防棕榈酸酯诱导的线粒体功能障碍和心肌细胞死亡。此外,HFD可抑制线粒体活性,导致受损的线粒体在心脏中积累,而这些现象在ACC2 iKO小鼠心脏中均得到了部分缓解。

(ACC2敲除小鼠高脂饮食喂养导致的心肌脂滴积累减少[剪头所指])

在机制上,ACC2 iKO阻断了HFD诱导的parkin下调。在线粒体刺激过程中,HFD喂养的对照小鼠心脏细胞线粒体上附着的parkin蛋白明显减少,而HFD喂养的ACC2 iKO小鼠心脏细胞线粒体上的parkin蛋白有部分恢复。

本研究结果表明,心肌FAO增加本身并不会引发心功能障碍,反而可以预防长期肥胖小鼠罹患心肌病。增强心肌脂肪酸氧化对高脂饮食诱导的肥胖的有益作用部分是通过维持parkin介导的线粒体功能来实现的。本研究还提示,靶向parkin依赖性的线粒体通路或可成为控制肥胖诱导性心肌病发展的有效策略。
