研究人员在Alzheimer's & Dementia杂志发文,其在一个大型统一的磁共振成像数据库(11项研究;10216名受试者)中,通过机器学习研究了典型衰老的脑萎缩模式。

衰老是一个复杂的,多因素的过程,其会异质性地影响大脑结构。多种神经退行性疾病与异常蛋白在大脑中的沉积相关,如阿尔茨海默病(AD),随着年龄的增长,其发病率有所增加,并会导致神经元的损伤和损失。但是在没有蛋白病变的情况下,老化本身可能也与突触和神经髓损失有关。

根据磁共振成像(MRI),在没有已知共病的情况下,脑老化似乎与灰质丢失有关,并可通过模式分析方法量化为 "脑年龄 "的衡量标准,这在某种程度上可与神经退行性疾病相关的萎缩模式相分离。

多项研究的证据表明,多种风险因素可能加速脑老化过程,功能上表现为加速的认知能力下降。大脑老化和神经退行性萎缩与影响记忆和执行功能的认知障碍有关,然而,每个人可能会影响不同的认知领域。例如,典型的脑老化和小血管缺血性疾病与执行功能和工作记忆的恶化有关。AD与神经纤维缠结(NFTs)中tau的异常沉积和神经斑块中的淀粉样蛋白β(Aβ)有关,通常会导致以遗忘为主的多领域综合征。

最近在机器学习和神经影像学方面的进展使得成像标记的发展成为可能,这些标记提供了一个总结性的衡量,来表征个体大脑结构或功能与典型的大脑老化轨迹的偏差。与这种模型的偏差可能反映疾病或对年龄相关条件的恢复能力的生物过程。

大脑变化的模式跨越多个层面,如大脑老化、白质疾病负担和神经退行性特征,其可以捕捉到个体之间的异质性,从而形成了与老化相关的疾病的多维概念,每个人都表现出独特的大脑改变模式。

最近,研究人员在Alzheimer's & Dementia杂志发文,其在一个大型统一的磁共振成像数据库(11项研究;10216名受试者)中,通过机器学习研究了典型衰老的脑萎缩模式与阿尔茨海默病(AD)、白质疾病、认知和AD神经病理学之间的关系。

研究人员在本次研究中计算了三种脑信号:脑年龄、AD样神经变性和白质增生(WMHs)。其测量了脑图表,并揭示了这些特征与认知和AD分子生物标志物的关系。

结果显示,在轻度认知障碍(MCI)/AD和认知正常(CN)的受试者中,WMHs与大脑高级老化、AD样萎缩、较差的认知和AD神经病理学有关。高WMH体积与CN受试者在约10年内发生脑老化和认知能力下降有关。WMHs与65岁后淀粉样蛋白β(Aβ)阳性的可能性增加一倍有关。此外,在MCI/AD中,脑老化、AD样萎缩和WMHs是比年代年龄更好的认知预测因子。

因此,该研究建立了脑图谱,量化了脑衰老的轨迹,使得人们能够系统地评价个人的脑衰老模式
