European Radiology:DECT在射血分数保留的心力衰竭患者中的应用

心脏磁共振(CMR)由于其良好的软组织分辨率,已经成为评估心肌纤维化的首选影像学方法。

尽管射血分数保留的心力衰竭(HFpEF)的发病率在世界范围内不断增加,但对其基本机制了解甚少,也没有有效的治疗措施。然而,在HFpEF中,心肌细胞和心脏细胞外基质(CEM)都会发生结构和功能的变化。心肌替代纤维/疤痕和心肌间质纤维化(MIF)是病理性心脏重构的关键组成部分。然而,将MIF评估与HFpEF联系起来,特别是缺血性HFpEF。

心脏磁共振(CMR)由于其良好的软组织分辨率,已经成为评估心肌纤维化的首选影像学方法。晚期钆增强 (LGE)表示心肌替代纤维化或心肌梗死(MI)后的瘢痕形成,根据增强方式可分为缺血和非缺血模式。T1 mapping定量测量心肌细胞外体积分数(ECV)可作为弥漫性MIF的替代标志。然而,由于扫描时间过长和禁忌症的存在,CMR在临床实践中的应用有限。一些研究表明,使用双能量计算机断层扫描(LIE-DECT)检测的晚期碘增强(LIE)和ECV与使用CMR测量的结果一致。迄今为止,只有少数研究报告了弥漫性MIF与非缺血性和缺血性HFpEF发生之间的关系。

近日,发表在European Radiology杂志的一项研究探讨了LIE-DECT测量的ECV增加与非缺血性和缺血性HFpEF之间的关系并进一步评估了其与心脏结构和功能的相关性为进一步阐明HFpEF的病理生理提供了参考依据

本研究共包括112名连续的HFpEF患者和80名连续的无心脏病的参与者(对照组),每位患者都接受了LIE-DECT检查。根据使用碘图检测到的LIE模式,所有患者被分为缺血组和非缺血组。在缺血性HFpEF组中计算缺血性瘢痕负担。碘图和血细胞计数分别用于测量非缺血性HFpEF组的ECV和缺血性HFpEF组非瘢痕心肌的远端ECV,收集了心脏结构和功能的变量。 

非缺血性HFpEF患者的ECV(n = 77)和缺血性HFpEF患者的远ECV(n = 35)都明显高于对照组(p < 0.001)。多变量逻辑回归分析显示,在调整了年龄、性别、体重指数、吸烟和饮酒后,较高的ECV/远ECV仍与非缺血性HFpEF和缺血性HFpEF有关(P<0.001)。在所有参与者中,ECV与心脏结构和功能变量之间正相关(P < 0.05)。亚组分析显示,在HFpEF亚组中,ECV/远端远程ECV和缺血性瘢痕负担与心力衰竭分类正相关(P < 0.05)。 


 74岁高血压心脏病患者两周内的LIE-DECT和LGE-CMR检查显示了非缺血性HFpEF的ECV的测量图像。血细胞比容值为41%。相应的心脏短轴位置的碘图(a)显示下壁的孤立的增强,被归类为非缺血,与LGE-CMR图像(b)一致。在人工测量ECV时,平均ECV被计算为35.8%(c),从ECV分布图中计算为34.0%(d)

本研究表明,使用LIE-DECT测量ECV是一项简单、方便、可重复的影像学技术,为HFpEF患者的心肌评估提供了有价值的信息。ECV的增加与非缺血性和缺血性HFpEF明显相关。使用LIE-DECT测量的定量ECV为追踪HFpEF不同亚型的病因和发病机制提供了基础。
