JACC:移植心脏血管病变检测的改进

本研究回顾了1993年至2009年小儿心脏移植研究的数据。移植肾功能异常被定义为显著收缩功能障碍(射血分数[EF]<45%)或限制性血流动力学特征的存在。额外的小儿血液流动力学指标为右心房压力(RAP)>12mmHg或肺毛细血管楔压(PCWP)≥15mmHg均被进行了分析。
    
在这项研究中,8,122次造影在3,120例患者中进行,70%的患者至少有1次血管造影。CAV造影发生率在2,5,10年时分别为5%,15%,和28%,多数疾病是轻微的。移植功能障碍的存在可以确定患者有移植损失的更大风险,甚至在轻度血管造影的儿童中(P<0.0001)。当RAP>12mmHg或PCWP>15mmHg时,足以检测患者移植损失的高风险,甚至是对于有轻度血管疾病的患者也可以检测。
    
仅有轻微的血管造影CAV的患者比中度或严重的疾病的患者有显著较好的结果。当EF<45%,RAP>12mmHg或PCWP>15mmHg时,即使只有轻微的造影血管病变的存在下,都可以确定儿童处于移植损失的高风险。
