IBD:西方和以肉为主的饮食模式与 IBD 发展的可能性更大有关

克罗恩病是一种原因不明的肠道炎症性疾病,在胃肠道的任何部位均可发生,但多发于末端回肠和右半结肠。本病和慢性非特异性溃疡性结肠炎两者统称为炎症性肠病(IBD)。

克罗恩病 (CD) 和溃疡性结肠炎 (UC)被统称为炎症性肠病 (IBD),它是一种慢性炎症性疾病。目前认为IBD 是由环境触发和肠道菌群失调的遗传易感个体共同导致的疾病,包括饮食异常导致的免疫反应增强。西化的生活方式已被认为有助于发展中国家IBD发病率的上升。相比之下,地中海饮食被广泛认为是具有抗炎作用的健康饮食模式,与晚发性 CD 风险显着降低相关。因此,人们认为饮食模式具有很大的临床意义。然而,关于营养和疾病发作的前瞻性数据有限。因此,本项研究旨在分析饮食模式和 IBD 发展的几率。

 

本项研究共纳入125445人,在14年的随访时间中,有224发展为UC,有97人发展为 CD。所有参与者在基线时都填写了与健康相关的饮食问卷 (FFQ)。研究人员对研究数据进行主成分分析 (PCA) 以推导出事后饮食模式,包括蛋白质评分、健康饮食指数、LifeLines 饮食评分 (LLDS) 和替代地中海饮食评分。在饮食模式、分数和 IBD 发展之间进行逻辑回归模型评估所有变量之间的相关性。

 

主成分分析总共确定了五种饮食模式,研究结果发现以大量摄入零食、预制餐、非酒精饮料、酱汁以及少量蔬菜和水果为特征的模式与 CD的出现的可能性较高有关(OR:1.16,95% CI:1.03-1.30,p =0.013 )。包含红肉、家禽和加工肉类的模式与 UC 发展的可能性增加有关(OR:1.11,95% CI:1.01-1.20,p = 0.023)。高饮食质量评分 (LLDS) 与 CD 风险降低相关(OR:0.95,95% CI:0.92-0.99,p = 0.009)。

 

本项研究通过大样本的前瞻性人群研究发现西方饮食模式与更大的 CD 发展可能性、食肉模式与 UC 发展相关,而相对较高的饮食质量 (LLDS) 对 CD 发展有保护作用。我们的研究强调了评估饮食模式以帮助预防普通人群 IBD 的重要性。
