Lancet:精氨酸刺激下和肽素测定用于尿崩症的诊断

研究认为,精氨酸刺激下和肽素测定是一种诊断尿崩症的新方法,该法具有较高的诊断准确率,可作为临床上诊断尿崩症的一种简便、新颖、安全的方法

尿崩症诊断具有极高的挑战性。最可靠的方法是高渗盐水刺激下和肽素(copeptin)测量。然而,该方法基于高钠血症诱导,需要密切监测血浆钠浓度。近日研究人员考察了精氨酸刺激下cepeptin测量结果准确性。

成年,因多尿(>50毫升/千克体重/天)转诊,或已知诊断为中枢性尿崩症或原发性多尿症患者参与研究,患者和健康对照组在基线和30、45、60、90和120 min时接受精氨酸刺激,同时测定血浆copeptin。研究的主要目的是确定血浆copeptin浓度的诊断准确性,以区分尿崩症和原发性烦渴症。所有参与者均监测试验的不良反应,使用视觉模拟评分(VAS)评定试验耐受性。

52名患者进入开发队列(12名[23%]患有完全尿崩症,9名[17%]有部分尿崩症,31名[60%]患有原发性烦渴症) ,20名健康成人和42名儿童为对照组。在合并的患者和对照组数据集后,健康成人对照组中精氨酸刺激的copeptin浓度中位数增加(从5.2pm增至9.8pm)和原发性烦渴症(从3.6增加到最大7.9 pm),但仅在尿崩症患者中最低(2.1pm)。在开发队列中,60分钟时3.5 pm为检测线,诊断的最高准确性为94%。在验证队列中,该临界值的准确性为86%。通过收集两个队列的数据,在60分钟时3.8pm copeptin的临界值达到93%的最佳准确率 ,灵敏度93%,特异性92%。该试验安全、耐受性好,尿崩症患者VAS评分中位数为3.5,原发性烦渴症为3,健康成人为1,健康儿童为1分。
