浙江大学新成果!有望改善阿尔兹海默患者认知功能

针对这一疾病,现有的全部治疗方法都无法阻止病情发展,患者一经确诊便无法治愈,并且日益恶化。

近日,浙江大学医学院附属邵逸夫医院精神卫生科主任医师陈炜团队研究发现,通过静息态功能磁共振(fMRI)技术能够有效改善阿尔兹海默病患者的认知功能,为治疗带来了曙光,其成果发表在《frontiers in aging neuroscience》和《psychiatry research》等国际期刊上。

“脑海中的橡皮擦”——这一别称形象地揭示出阿尔茨海默病的特点:作为一种常见于老年人的神经退行性脑部疾病,其将使患者失去记忆、失去生活自理能力、失去生命的光彩,为无数个体和家庭带来痛苦。针对这一疾病,现有的全部治疗方法都无法阻止病情发展,患者一经确诊便无法治愈,并且日益恶化。

陈炜团队介绍,前期的临床研究已证实,重复经颅磁刺激可以改善患者的认知功能和行为。团队利用静息态功能磁共振(fMRI)技术,对轻、中度患者进行顶叶—海马功能连接重复经颅磁刺激治疗,重点刺激大脑深部作为记忆关键脑区的海马脑区。相较于传统技术,这一疗法的进步之处在于利用功能影像技术的发展,通过磁共振功能连接的方法,成功实现大脑皮层刺激与深部脑区的连接,解决了传统治疗无法刺激深部脑区的难题。

研究过程中,陈炜团队对患者进行了为期2周、共计10次的刺激,通过分析认知心理量表评分、静息态功能磁共振功能连接的变化,考察重复经颅磁刺激的治疗效果及机制。

结果显示,仅在两周内,基于功能连接的重复经颅磁刺激就能对轻、中度患者的认知功能有所改善。具体来说,接受了治疗的真刺激组在简明心理状况、费城语言学习测验等测试中分值显著提高,与并未受到实际疗效的假刺激组比较有显著差异;即该新型疗法下的患者在语言能力、学习能力等方面表现较好,说明其认知能力有所回升。

同时研究发现,患者默认网络的功能连接增强,提示治疗后患者的脑功能得到改善,改善越显著说明临床效果越好。

团队的研究成果已接连发表在《frontiers in aging neuroscience》和《psychiatry research》等国际期刊。两篇论文的第一作者分别是邵逸夫医院精神卫生科研究生贾艳利和医师魏丽丽,通讯作者是陈炜。研究已得到国家自然科学基金、国家重点研发计划、浙江省重点研发计划和杭州市科学技术重大项目的支持。

同时研究人员认为,还需要继续延长治疗时间,以巩固疗效。目前,陈炜课题组正在开展多中心临床研究,希望探索更优化的治疗方法,提高临床效果。
