正常组织基因启动子甲基化在癌症风险中的作用尚不明确。近日,内科学权威杂志Annals of Internal Medicine上发表了一篇研究文章,该研究评估正常组织BRCA1甲基化与卵巢癌风险之间的关系。

在这2个病例-对照(初始和验证)研究中,以挪威的2家医院(病人)和基于人群的研究(对照组)为基础,在初始研究中包含了934名患者和1698名对照参与者;在验证研究中包含了607名患者和1984名对照参与者。所有患者在化疗前均进行了血液采样。采用甲基化特异性定量聚合酶链反应检测了白细胞(WBC)BRCA1基因启动子甲基化水平,研究人员比较了人群对照者与卵巢癌患者之间的甲基化阳性百分比,卵巢癌患者中包含了高级别浆液性卵巢癌(HGSOC)的亚组。

在初始研究中,卵巢癌患者BRCA1甲基化相比于对照者更频繁(6.4% vs. 4.2%;年龄调整后的比值比(OR)为1.83(95%可信区间(CI)为1.27-2.63))。但是较高的甲基化水平仅限于HGSOC患者(9.6%;OR为2.91[CI为1.85-4.56]),对应的非浆液性和低级别浆液性卵巢癌(LGSOC)患者分别为5.1%和4%。这些研究结果在验证研究中得到了重复(HGSOC患者的甲基化阳性率为9.1% vs. 对照参与者为4.3%,OR为2.22[CI为1.40-3.52],非浆液性卵巢癌患者的4.1%,LGSOC患者为2.7%)。这些结果不受肿瘤负担、、存储时间或白细胞组成的影响。在对年轻妇女和新生儿的单独分析中,BRCA1基因甲基化检测分别为4.1%(CI为1.8%-6.4%)和7%(CI为5%-9.1%)。

由此可见,正常组织中BRCA1基因启动子甲基化水平与HGSOC风险呈正相关性。
