肺动脉高压(PAH)是一种罕见的疾病,可因右心衰竭导致过早死亡。它与红细胞分布宽度(RDW)增加密切相关,RDW与几种铁状态生物标志物有关。高RDW预示早期铁缺乏症或铁缺乏性贫血。近日,呼吸疾病领域权威杂志Eur Respir J上发表了一篇研究文章,这项研究调查了RDW增加是否与PAH之间存在因果关系。

研究人员采用了两个样本的孟德尔随机(MR)方法探究了遗传倾向较高的RDW是否增加了PAH的发生几率。研究人员使用所有可用的全基因组显著的RDW变体(n=179)和五个通过全身铁状态起作用的全基因组显著RDW变体进行了主要和次要MR分析。

在多中心病例对照研究中(病例组N=642,对照组N=15889),研究人员观察到RDW与PAH之间的相关性(OR=1.90,95%CI=1.80–2.01)。基于RDW GWAS报告的估计值或该研究的数据,主要的MR分析具有足够的能力来检测(OR)1.25至1.52或更大之间的因果效应。在主要(因果OR=1.07,95%CI=0.92–1.24)或次要(因果OR=1.09,95%CI=0.77–1.54)MR分析中,均没有证据表明RDW与PAH之间存在因果关系。
