血小板增多症和炎症是动脉粥样硬化发病机制中的重要因素。血小板淋巴细胞比率(PLR)是一种结合了这些参数的生物标志,已被证实与心血管疾病(CVD)相关。近日,心血管领域权威杂志International Journal of Cardiology上发表了一篇研究文章,该研究的目的是明确PLR是否与高危患者的冠状动脉疾病(CAD)相关性,以及这种相关性是否受年龄的影响。

研究人员评估了连续就诊进行冠状动脉造影的患者(n=822)。其中冠心病患者608例,正常冠状动脉者214例。患者被分为早发CAD(年龄<55岁)和非早发CAD(年龄≥55岁)。高和低PLR组被定义为入院PLR分别位于最高(≥146.7)和最低(<146.7)三分位数。研究人员采用多元logistic回归对传统心血管危险因素进行调整。

在单变量分析中,高PLR与早发CAD呈显著负相关(比值比为0.45,95%可信区间为0.23-0.87,P=0.017),而其与CAD之间的相关在老年患者中并没有达到统计学显著性(比值比为1.32,95%可信区间为0.89-1.97,P=0.170)。对传统的危险因素进行调整后,高PLR与老年患者CAD增加显著相关(比值比为2.22,95%可信区间为1.28-3.82,P=0.004),而过早CAD有所降低(比值比为0.31,95%可信区间为0.11-0.87,P=0.026)。
