World J Pediatr：小儿心力衰竭入院血压百分位与院内死亡风险的关联

该研究首次确立小儿心力衰竭血压百分位风险分层阈值，为临床血流动力学管理提供量化参考。

深度解析医学证据，DeepEvidence为你支撑决策   小儿心力衰竭是儿童群体中高病死率的心血管综合征，其入院血压的预后价值尚未明确，临床也缺乏循证风险分层阈值。为填补这一空白，国内 30 家医疗中心开展一项多中心回顾性队列研究，纳入 2013-2022 年 2545 例确诊小儿心力衰竭患儿，探究校正年龄、性别、身高后的入院血压百分位与院内死亡的关联，为临床精准干预提供依据。     研究将收缩压、舒张压转换为对应百分位，通过限制性立方样条、机器学习协变量筛选及分层分析开展研究，整体患儿院内病死率为 4.20%，死亡组收缩压、舒张压百分位均显著低于存活组。结果显示，收缩压百分位与院内死亡呈 U 型关联，最低死亡风险对应第 63.8 百分位，第 57 至 91 百分位为低风险区间；低于第 57 百分位时死亡风险升高 86%，高于第 91 百分位风险升高 69%，该关联主要由婴幼儿群体驱动。舒张压百分位则与死亡呈线性负相关，第 83 至 100 百分位为低风险区间，低于第 83 百分位时死亡风险升高 65%。   图：研究概述   性别
