男同性恋和双性恋者接受HIV接触前预防后，细菌性性传播感染风险增加

新的迹象表明男同性恋和双性恋者接受HIV接触前预防(PrEP)会导致细菌性性传播感染(STIs)风险增加，近日研究人员就该问题进行了深入研究。

PrEPX研究在澳洲开展，2981名志愿者参与，随访至少1年，参与者每日口服替诺福韦和恩替他滨用于HIV接触前预防、每季度接受HIV、STI以及临床监测。研究的主要终点为衣原体、淋病或梅毒的发病率。

参与者平均年龄34岁，98.5%为男同性恋和双性恋者，29%在研究前接受过PrEP，2892人(97.0%)完成随访。在随后的1.1年中，1427名(48%)参与者发生了2928起STIs（衣原体1434例，淋病1242例，梅毒252例），每100随访年中发生91.9例STI。多因素分析显示，更小的年龄、更多的伴侣数量以及群体性行为导致STI风险增加，但使用避孕套可规避风险。比较研究前后数据发现，在PrEP前，STI每100随访年发生69.5例，PrEP后增加至98.4例，多因素调整后，PrEP后所有STI（1.12）以及衣原体（1.17）感染风险增加。

研究证实，男同性恋和双性恋者接受HIV接触前预防后，细菌性性传播感染风险增加。
