Thorax:哮喘患者口服和吸入皮质类固醇导致骨质疏松和脆性骨折的风险

OCS或ICS是哮喘患者骨骼健康的独立危险因素。建议以尽可能低的类固醇激素维持哮喘控制。

吸入性皮质类固醇(ICS)和口服皮质类固醇(OCS)广泛用于哮喘的治疗。然而,哮喘患者中由皮质类固醇引起的骨质疏松和脆性骨折(FF)的风险尚未明确。

近日,胸部疾病领域权威杂志Thorax上发表了一篇研究文章,研究人员使用来自临床实践研究数据链接(CPRD)和医院病情统计(HES)数据库的链接数据进行了两个嵌套的病例对照研究。使用哮喘队列,研究人员​​分别确定了骨质疏松或FF患者以及性别、年龄和实践匹配的对照者。研究人员采用条件Logistic回归用于确定ICS和OCS暴露与骨质疏松或FF的风险之间的关联,并计算了接受至少一种双膦酸盐治疗患者的患病率。

过去一年中,OCS/ICS的累积剂量和数量与骨质疏松症或FF的风险之间存在剂量反应关系。调整混杂因素后,接受更多OCS处方(≥9 vs. 0)的患者发生骨质疏松症和FF的风险分别增加了4.50(95%CI为3.21至6.11)和2.16(95%CI为1.56至3.32)。对于ICS(≥11 vs. 0),OR分别为1.60(95%CI为1.22至2.10)和1.31(95%CI为1.02至1.68)。累积剂量具有类似的影响,接受更多OCS或ICS的患者风险更大。服用≥9OCS和至少一种双膦酸盐治疗的患者发生骨质疏松症和FF的患病率分别为50.6%和48.4%。

该研究的结果表明,OCS或ICS是哮喘患者骨骼健康的独立危险因素。建议以尽可能低的类固醇激素维持哮喘控制。
