Hs-cTnT水平与轻微卒中或TIA后三年内发生的MACE有关,可能有助于识别高危人群,否则根据ABCD2评分认为风险较低。

急性缺血性卒中(AIS)或短暂性脑缺血发作(TIA)的患者有复发血管事件的高风险。一些临床预测方案,如 "年龄、血压、临床特征、症状持续时间、糖尿病"(ABCD2)评分已被提出,以指导患者的临床分诊。

虽然最初是为了TIA后的早期分流,但有人认为ABCD分数越高,缺血性卒中后复发血管事件的风险也越大。 然而,脑血管事件后未来血管风险的个体分层仍然是有限的,个体风险可能有很大差异。

人们越来越认识到,基于血液的生物标志物通过捕捉生物相关的途径,可能有助于提供常规风险因素之外的个体风险估计。

最近的观察强调了高敏心肌肌钙蛋白(hs-cTn)水平在这方面的潜在作用。hs-cTnT升高超过检测特定的99百分位参考上限(URL)的心肌损伤在中风后很常见,并与不良的长期预后有关。

在普通人群以及心房颤动患者中,较高的hs-cTn水平与血栓栓塞和主要不良心血管事件(MACE)的风险增加有关。

然而,在最近的缺血性卒中或TIA患者中,这种关联还没有得到很好的证实。

通常情况下,抗血栓治疗和危险因素控制被认为适用于所有卒中后的患者,但某些患者是否能从强化的二级预防措施中获益仍是未知数。

最近完成的INSPiRE-TMS试验(INtensified Secondary Prevention intending a Reduction of recurrent Events after Transient ischaemic attack and Minor Stroke)表明,与常规治疗相比,随机接受多成分二级预防支持方案的患者在危险因素控制方面有明显改善。

然而,这并没有转化为这些患者后续MACE的显著减少。然而,亚组分析表明,风险较高的ABCD2得分的患者可能会从研究干预中受益。

通过使用INSPiRE-TMS的数据,德国柏林大学的Simon Hellwig等人探究了:
1)心肌损伤的存在(hs-cTnT水平高于URL)是否与轻微卒中或TIA患者的复发性MACE有关;
2)根据ABCD2评分,这种关联是否在被认为具有较低或较高血管风险的患者中保持;
3)INSPiRE-TMS中测试的多成分支持计划的效果是否因hs-cTnT水平而改变。

INSPiRE-TMS是一项随机对照试验,将轻微中风或TIA患者分配到强化支持计划或常规护理中。在这项事后分析中,利用hs-cTnT水平(第5代,罗氏,99百分位数参考上限[URL]14ng/L)对参与者进行分类。使用ABCD2评分对血管风险进行分层(低风险:0-5 vs. 高风险:6-7)。使用协变量调整和倾向评分匹配(PSM)对hs-cTnT和MACE(卒中/非致命冠状动脉事件/血管死亡)之间的关系进行了Cox比例危险回归。

在889名患者中(平均年龄70岁,37%为女性),在平均3.2年的随访期间,有153名患者(17.2%)发生MACE。Hs-cTnT与MACE相关(9.3%/年>URL与4.4%/年≤URL,调整后的HR为1.63[95%CI 1.13-2.35],调整后的HR(Q4与Q1)为2.57[95%CI 1.35-4.97],调整后的HR(对数转换后)为2.31[95%CI 1.37-3.89])。

这种关联在PSM后仍然存在(调整后的HR为1.76[95%CI 1.14-2.72])。hs-cTnT和ABCD2类别之间对MACE的发生有显著的交互作用(pinteraction =0.04)。

在低风险类别中,hs-cTnT>URL的患者MACE发生率为9.5%/年,高于≤URL者(3.8%/年),与高风险类别的总发生率相似。

这个研究的重要意义在于发现了:Hs-cTnT水平与轻微卒中或TIA后三年内发生的MACE有关,可能有助于识别高危人群,否则根据ABCD2评分认为风险较低。如果在独立的验证研究中得到证实,这可能需要对hs-cTnT升高的卒中患者加强二级预防措施和心脏诊断
