血小板与白细胞计数 (PWR) 的比率可用作 CN-AML 患者总生存期和无事件生存期的独立预测因子

最近有研究报道血小板与白细胞比率(PWR)是急性早幼粒细胞白血病的独立预后预测因子。急性髓性白血病 (AML) 在疾病诊断时通常表现为血小板计数和白细胞计数 异常。然而,PWR 对细胞遗传学正常的 AML (CN-AML) 的临床影响仍不清楚。因此,一研究团队评估了其对 CN-AML 患者的预后影响。

研究人员记录了疾病诊断时的临床信息,并计算了 338 例 CN-AML 患者的血小板计数与白细胞计数的比率。主要目的是研究 PWR 对 CN-AML 患者总生存期 (OS) 的预后影响。OS 的衡量标准是从疾病诊断到任何原因导致的死亡或对最后一次随访时存活的患者进行审查的日期。

为了评估 PWR 的预后价值,研究人员根据 PWR 值的三个相等四分位数(Q1:<33.3%,Q2:33.3%–66.6%,Q3:>66.6%)将患者分为低、中和高组。在包括白细胞计数、年龄和NPM1、FLT3 -ITD、CEBPA和DNMT3A突变基因在内的公认预测因素的背景下研究了 PWR 的独立预后价值。接受者操作特征 (ROC) 曲线用于评估其预后预测的性能。

研究结果提示较高的 PWR 具有较高水平的血小板计数和较低水平的白细胞计数、骨髓原始细胞百分比、FLT3 -ITD 和NPM1突变。单独的 PWR 和联合分子生物标志物的生存预测性能相当。此外,PWR 对分子生物标志物具有额外的预后信息。最后,PWR 与 CN-AML 患者良好的总生存期和无事件生存期相关,与遗传亚型和临床参数无关。

但仍存在一些局限性。首先,CN-AML 患者是从单一中心回顾性招募的,因此可能存在选择偏差。其次只检查了FLT3 -ITD、NPM1、 CEBPA和DNMT3A突变的基因,不能排除其他突变基因,如IDH1IDH2ASXL1TET2突变,这也会混淆 PWR 在 AML 患者中的预后价值。第三,仍需要进一步的大规模和前瞻性研究来验证 PWR 的预后价值。因此,在应用此研究结果时仍需谨慎。

总的来说,该研究发现血小板与白细胞计数 (PWR) 的比率可用作 CN-AML 患者总生存期和无事件生存期的独立预测因子。
