    中枢神经系统恶性肿瘤发病率越来越高,且5年相对存活率仅为35.8%。包括WHOII级和III级的低级神经胶质瘤(LGG)为异质性肿瘤。越来越多的证据表明,相同级别的LGG患者在分子特征、治疗反应和临床预后上差异很大。最近的研究提出了几种LGG分子改变直接影响新疗法的发展和临床预后。因此,临床医生越来越依靠遗传分类来指导新诊断的LGG的治疗决策。

      DNA拷贝数变异(CNV)是定量的结构重排,包括重复,缺失和高阶扩增。在整个癌症发生发展进程中均伴随着DNA拷贝数的变化。癌症基因组图谱(TCGA)队列的综合分析确定了三种DNA拷贝数类别(CN1-CN3)。

    CN2亚型与最短总生存期(OS)相关。CN1亚型和CN3亚型与相对较长的OS相关。CN2亚型中的大多数LGG具有野生型IDH。CN3亚型中的LGG几乎均具有IDH突变和1p / 19q密码子缺失。CN1亚型的LGG有较多的IDH突变且无1p / 19q密码缺失。放射基因组学提供了一种实用的结合基因型和表型特征来开发影像学生物标志物的方法。然而,关于MR特征和DNA拷贝数亚型之间的关系的研究十分有限。


      近日,发表在European Radiology杂志的一项研究应用基于MR的术前列线图预测了LGG患者(CN1-CN3)DNA拷贝数亚型中,证明了低级别胶质瘤MR特点与DNA拷贝数亚型的关系。

    分析了患者的总生存期(OS)数据。回顾性分析170例患者的MRI数据。通过单因素和多因素回归分析评价两者的相关性。

    CN2亚型与最短中位OS相关(CN2亚型vs.其他:46.8 vs. 221.7个月,p <0.05)。CN2亚型的时间依赖性受试者工作特征为1年生存期为0.80(95%CI:0.74-0.85)、2年生存期为0.80(95%CI:0.75-0.85)以及3年生存期为0.77(95%CI:0.73-0.83)。在多变量分析中,出血(OR:0.118; p <0.001; 95%CI:0.037-0.376)、边界不清(OR:4.592; p <0.001; 95%CI:1.965-10.730)、结节外生长(OR:0.247) ; p = 0.006; 95%CI:0.091-0.671)、体积≥60 cm3(OR:4.734.256; p <0.001; 95%CI:2.051-10.924)与CN1亚型有相关性(AUC:0.781)。CE肿瘤比例(OR:5.905; p = 0.007; 95%CI:1.622-21.493)、结节外生长(OR:9.047; p = 0.001; 95%CI:2.349-34.846)、宽度≥中位数(OR:0.231; p = 0.049; 95%CI:0.054-0.998)和高度≥中位数(OR:0.192; p = 0.023; 95%CI:0.046-0.799)与CN2亚型有相关性(AUC:0.854)。坏死/囊变(OR:6.128; p = 0.007; 95%CI:1.635-22.968)、出血(OR:5.752; p = 0.002; 95%CI:1.953-16.942)、边界不清(OR:0.164; p < 0.001; 95%CI:0.063-0.427)和体积≥中位数(OR:4.422; p <0.001; 95%CI:1.925-10.160)与CN3亚型有相关性(AUC:0.808)。所有三个列线图均显示出良好的区分度和校准度。决策曲线分析支持所有列线图在临床上有用性。十倍交叉验证的平均准确度分别为0.680(CN1)、0.794(CN2)和0.894(CN3)。
