研究人员选择了接受前列腺根治性切除术的218例患者(102名N+;116名N0)的苏木精-伊红(HE)染色原发肿瘤玻片,这些患者的Gleason评分、肿瘤大小、静脉侵袭、嗜神经侵袭和年龄均匹配,这些患者用于训练CNN和评估其预测淋巴结状态的能力。

研究人员用同样的数据训练了10个模型,平均接收者操作特征曲线下面积(AUROC)为0.68(95%CI:0.678-0.682),平均平衡准确率为61.37%(95%CI:60.05-62.69%)。平均灵敏度和特异性分别为53.09%(95%CI:49.77-56.41%)和69.65%(95%CI:68.21-71.1%)。这些结果通过交叉验证得到了证实。在来自N+样本的图像切片上,LNM预测的概率得分显著更高(平均N+概率得分:0.58±0.17 vs N0概率得分:0.47±0.15,p=0.0016)。在多变量分析中,CNN的概率评分(OR 1.04‰,95%CI:1.02-1.08,p=0.04)和淋巴血管入侵(OR 11.73,95%CI:3.96-35.7,p<0.0001)证明是LNM的独立预测因子。

最后,研究人员指出,基于CNN的图像分析作为一种潜在的新型低成本方法,可直接从HE组织学中提取相关的预后信息来预测前列腺癌患者的淋巴结状态。该技术可能有助于改善淋巴结状态的预测
