近日,心脏病领域权威杂志Heart上发表了一篇研究文章,该研究旨在评估独居的冠心病(CAD)患者与家人居住在一起的CAD患者相比血管(CV)结局。

稳定冠状动脉疾病患者的前瞻性纵向观察研究(CLARIFY)纳入了稳定性冠心病的门诊患者。CLARIFY在2009年11月至2010年7月期间招募了45个国家的参与者,并进行了5年的随访。在基线时记录了患者的生活情况。主要结局是主要不良血管事件(MACE)的复合事件,定义为心血管死亡、心肌梗死(MI)和卒中

在32367例患者中,3648例(11.3%)患者独居。多变量调整后,独居患者与其他患者相比,MACE无残余差异(HR为1.04,95%CI为0.92-1.18,p=0.52);然而,性别暴露效应存在显著的异质性(相互作用P<0.01)。具体而言,独居的男性患MACE的风险较高(HR为1.17,95%CI为1.002-1.36,P=0.047),而独居的女性则不然(HR为0.82,95%CI为0.65-1.04,P=0.1),主要是由性别对MI的异质性影响所致(相互作用P=0.006)。虽然年龄对MI(相互作用P=0.046)和卒中(相互作用P=0.05)可能有不同的影响,但年龄组(相互作用P=0.3)对MACE没有调节效应。
