Radiology:对于阿尔兹海默病&轻度认知障碍,哪种检查更敏感呢?

本研究共45例AD患者、20例MCI患者和11例HC受试者,并分析受试者PER/MRI的脑血流量(CBF,脉冲动脉自旋标记[ASL] MRI)、葡萄糖代谢(18氟脱氧核糖核酸[FDG] PET)、灰质(GM)体积(结构T1加权MRI)。经过预处理、基于体素方差分析,感兴趣区体积和独立成分分析进行比较CBF和葡萄糖代谢。

结果为,与受试者相比,AD患者在楔前叶、额叶、颞叶和枕叶皮层在低灌注和低代谢分布、定量指标间有高度交叉。MCI患者与HC受试者相比,仅有FDG PET显示楔前叶具有低灌注和相应成分。感兴趣区体积分析显示AD患者整体GM CBF(42 mL/100 g minute ± 8 vs 49 mL/100 g per minute ± 7,; P = .035)和FDG摄入量要明显低于HC受试者(0.8 ± 0.1 vs 1 ± 0.1, respectively; P < .001)。

本研究表明,在AD患者中,脉冲ASL MRI显示区域、程度和量上具有与18F-FDG PET相似的异常。在MCI患者中,仅有18F-FDG PET显示楔前叶出现代谢异常。这表明与脉冲ASL MRI序列相比,18F-FDG PET更能检出临床前AD。
