NEJM:急性脑损伤患者脑活动与指令反应

研究发现,15%的急性脑损伤患者对运动指令缺乏行为反应,这是脑活动与指令反应间存在分离的证据

临床上反应迟钝的患者,脑电图学(EEG)可以检测到大脑对口头运动指令的反应。在脑损伤后的头几天,运动行为和大脑活动之间的分离和其对预后的意义还没有得到很好的理解。

急性脑损伤,并且对口头命令无反应患者参与研究。通过脑电图记录检测大脑激活,并观察命令响应。定义12个月时格拉斯哥评分(GOSE)1到8的患者为预后良好。

104例无反应的患者中有16例(15%)在伤后4天,脑电图(EEG)检测到脑活动。16例患者中有8例(50%)和88例无脑活动患者(26%)中有23例(26%)的病情得到改善,上述患者在出院前对指令有响应。在12个月时,16例脑激活患者中有7例(44%)和84例(14%)无脑激活者中有12例(14%)GOS-E水平在4或更高水平,表明患者在8小时内具有独立功能的能力(优势比4.6)。

研究发现,15%的急性脑损伤患者对运动指令缺乏行为反应,这是脑活动与指令反应间存在分离的证据。
