流行病学资料看沉重疾病负担

流行病学数据显示,2017年卒中、缺血性心脏病和慢阻肺是国人死亡的前三位杀手,卒中年龄标化患病率为1114.8/10万人;卒中幸存者最常见的危险因素为高血压、吸引和饮酒。关于卒中类型,最常见的类型是缺血性卒中和脑出血(ICH)。从1990年到2017年,年龄校正的卒中死亡率下降33.5%,每10万人中卒中导致的伤残调整寿命年减少33.1%。这在很大程度上与卒中相关危险因素管理尤其是血压管理方面的进步分不开。

血压管理在卒中防治中的重要性

血压管理应贯穿整个卒中方式的全过程。在发病前,做好高危人群的血压管理,将血压控制在140/90 mm Hg以内,有助于实现良好的一级预防。在卒中出院后,血压管理是二级预防的重要手段,控制目标值同样为140/90 mm Hg。那么,在急性卒中住院期间,我们应将血压控制在什么水平?不同类型的卒中是否设定不同的目标水平?

急性卒中发生后血压通常升高,这可能与脑血管自动调剂破坏、自主神经系统受损、神经内分泌因素、头痛、泌尿系梗阻、感染和情绪心理等多种因素有关。这种急性血压增高,对卒中患者是具有保护性还是破坏性作用?实际上,研究发现,脑出血急性期血压增高对患者预后具有重要预测价值,但卒中类型不同对预后的影响不同。具体来说,脑出血时血压增高水平与预后呈线性相关,血压越高,预后越差,死亡率越高;缺血性脑卒中(IS)时基线收缩压与短期和长期死亡率呈U型相关,发生不良转归最低风险时的收缩压为150 mm Hg,过低和过高均与较差转归相关。对上述两种常见类型卒中发生后血压自然变化过程的对比分析发现,缺血性卒中时收缩压仅轻度增高,出血性卒中血压升高更显著,尤以发病24 h内为著。

▣ 脑出血急性期血压管理目标

脑出血后,血压显著增高,且随着血压的增高,血肿扩大发生率显著增加,而血肿扩大是不良预后的独立危险因素;脑出血早期血压升高与预后不良相关。良好的血压管理可能是改善脑出血患者预后行之有效的方法。

对急性脑出血患者,血压管理(降压治疗)是一把双刃剑,积极降压有助于防止血肿扩大,但可增加继发缺血风险。2007年美国脑出血治疗指南推荐,若脑出血患者收缩压>200 mm Hg或平均动脉压>150 mm Hg,可积极降压;若收缩压>180 mm Hg或平均动脉压>130 mm Hg,则应适度降压,将血压控制在160/90 mm Hg。随后INTERACT研究显示,对急性脑出血患者,与180 mm Hg相比,早期强化降压将收缩压控制在140 mm Hg基本安全且可降低血肿增长。相关荟萃分析也显示,强化降压总体来说安全。正是基于此,2010年,美国脑出血指南推荐脑出血后收缩压在150~220 mm Hg,积极控制收缩压在140 mm Hg以下基本安全。


INTERACT-2研究显示,强化降压(将收缩压降至140 mm Hg)可改善生存者的躯体功能恢复和健康相关生活质量。因此,2015年美国脑出血指南明确指出,脑出血患者收缩压在150~220 mm Hg之间若无禁忌证,将患者血压快速降至140 mm Hg是安全的,且可有效改善功能预后。不过,随后开展ATACH-2研究却不支持将脑出血患者的收缩压快速降至110~139 mm Hg能更好地改善功能预后。对INTERACT2研究的事后分析显示,积极强化降压可能使不同基础血压水平的患者获益,将收缩压控制在130~139 mm Hg可最大获益。当然,这还有待进一步在临床实践中证实。基于相关研究荟萃分析的结果,目前,强化降压能否改善预后仍不明确。因此,对脑出血患者,适度降压能改善预后,血压管理应重视时间窗问题。

▣ AIS的血压管理

对AIS患者而言,降压治疗同样是一把双刃剑,积极降压有助于降低出血转化和减轻水肿,但增加梗死面积扩大及侧枝减少的风险。因此,其血压管理需注意处理好维持脑血流与保证急性期缺血性卒中良好结局(避免脑水肿、出血性转化及继发性心血管事件)的平衡。首先,在AIS发病后几天启动降压治疗最合适?相关研究发现,AIS早期(72 h内)降压治疗并不降低发病后3个月或出院后的死亡率、不能改善预后。因此,中国AIS诊治指南推荐,缺血性脑卒中后24 h内血压升高的患者应谨慎处理,先处理紧张焦虑、疼痛、恶心呕吐及颅内压增高等情况。若血压持续升高至收缩压≥200 mm Hg或舒张压≥110 mm Hg或伴有严重心功能不全、主动脉夹层、高血压脑病者可予以降压治疗并密切观察血压变化,治疗时建议使用微量输液泵给予静脉药物,避免使用引起血压急剧下降的药物。当卒中后病情稳定后,若血压持续≥140/90 mm Hg,无禁忌证可于发病数天后恢复使用发病前的降压药物。



研究发现,静脉溶栓者溶栓前及溶栓后高血压均与症状性出血转化及预后不良呈正相关。ENCHANTED研究表明,静脉溶栓患者早期降压治疗安全,并不能改善患者预后,但可降低颅内出血发生率。

对动脉取栓者,取栓后血压升高与死亡率及残疾率增加相关;取栓治疗前后24 h内的血压变异与症状性颅内出血显著相关。因此,维持血管内治疗后血压稳定对减少症状性颅内出血有重要意义。目前,尚无前瞻性随机试验对血管内取栓患者血压管理目标进行研究。

总之,AIS患者早期积极降压治疗安全,但并不能改善预后及降低复发。临床实践中,应重视血压管理在降低AIS后出血转化风险的作用,对静脉溶栓及血管内治疗者血压管理则应更积极。此外,血压的管理不仅要降低血压的绝对值,还要降低血压变异性。
