Cephalalgia：无创迷走神经刺激在急性和发作性慢性丛集性头痛中的不同疗效

两项随机，双盲，假手术对照试验（ACT1，ACT2）评估了无创迷走神经刺激（nVNS）作为丛集性头痛的急性治疗方法。我们分析了合并的ACT1/ACT2数据，以提高统计能力并深入了解nVNS在发作性和慢性丛集性头痛中的不同疗效。

 

使用固定效应模型汇总从ACT1和ACT2提取的数据。主要结局指标是每项研究的主要终点。这是ACT1的首次治疗发作从中度（2），严重（3）或非常严重（4）的疼痛强度改善为轻度（1）或无（0）的参与者的比例，以及治疗后疼痛发作的比例ACT2的强度从2-4提高到0。

 

结果显示，汇集的人群包括来自nVNS（n = 108）和假（n = 117）组的ACT1（n = 133）和ACT2（n = 92）的225名参与者（发作期：n = 112；慢性病：n = 113）。治疗组与丛集性头痛亚型之间存在相互作用（p <0.05）。在发作性发作中，nVNS优于假手术，但在慢性丛集性头痛中则不然（两个终点p <0.01）。由于不良事件，4名患者中止了研究。

 

综上所述，该研究结果表明，nVNS是发作性丛集性头痛的一种耐受良好且有效的急性治疗方法。
