Carbohydr Polym:化疗-光动力联合疗法治疗银屑病:壳聚糖/透明质酸纳米凝胶共传递甲氨蝶呤和5-氨基乙酰丙酸

该研究首次开发了一种自组装的CS-HA纳米凝胶,将MTX和ALA用于局部联合化疗/光动力疗法,从而将副作用降至最低,并提高了银屑病的治疗效果。

     银屑病是一种以角质形成细胞(KC)过度增殖为特征的慢性T细胞介导的炎症性皮肤病。全球大约2-3%的人口患有银屑病。甲氨蝶呤(MTX)具有抗增殖和免疫调节作用,被广泛用作皮肤科化疗药物。口服甲氨蝶呤是治疗中重度银屑病的一线低成本疗法。

    不幸的是,口服MTX的全身毒性表现为肝毒性和肾毒性,限制了其应用。因此,局部应用甲氨蝶呤是一种潜在的替代疗法,因为它在作用部位有效,减轻了全身不良反应。然而,由于甲氨蝶呤的亲水性(logp−1.85)和过度角化银屑病斑块,外用MTX的

皮肤渗透性有限。因此,必须研制一种新型的甲氨蝶呤透皮给药系统来增强甲氨蝶呤的皮肤渗透性。此外,银屑病是一种复杂的疾病,对MTX或其他单一疗法没有足够的反应。联合疗法已被提出用来优化银屑病的治疗结果。

     光动力疗法(PDT)在包括银屑病在内的浅表皮肤病患者中因其快速疗效、非侵入性和良好的美容效果而受到高度重视。基于5-氨基乙酰丙酸(ALA)的光动力疗法(ALA-PDT)已得到美国食品药品监督管理局(FDA)的批准。外源ALA在细胞内被代谢成光敏剂(PS)原卟啉IX(PPIX),积累的PPIX在光照后产生活性氧(ROS),导致靶组织凋亡和坏死。然而,ALA的亲水性阻碍了其对银屑病皮肤角质层(SC)的渗透,从而抑制了ALA-PDT治疗银屑病的效果。因此,提高PDT治疗银屑病疗效的途径是非常可取的。PDT与化疗或免疫疗法相结合以提高疗效。利用MTX和ALA-PDT治疗银屑病的优势,MTX联合ALA-PDT局部治疗银屑病是一种很有前途的治疗方法。然而,该载体同时包裹MTX和ALA并增强它们的皮肤通透性仍然是一个挑战。

      壳聚糖(CS)和透明质酸(HA)这两种具有良好生物相容性和生物降解性的多糖被用来制备纳米载体来包裹局部治疗皮肤病的药物。它们可以在治疗过程中发挥积极作用,被称为“使能辅料”。CS是一种阳离子多糖,是一种局部抗菌剂和渗透促进剂。HA是一种天然的阴离子型糖胺聚糖,是皮肤细胞外基质的主要成分。羟基磷灰石水凝胶已被广泛研究用于治疗皮肤病。CD44是透明质酸(HA)受体之一,在银屑病患者皮肤中高度表达,表明CD44可以作为主动靶向纳米载体治疗银屑病的潜在靶点。HA介导的经皮给药表现出增强的渗透性和靶向抑制银屑病皮肤炎症。有趣的是,CS和HA可以通过温和的离子凝胶自组装形成仿生纳米凝胶(CS/HA NGS),CS/HA NGS结合了CS的细胞摄取增强能力和CD44受体介导的HA内化,从而实现了对银屑病的靶向输送。此外,CS/HA NGS可以通过持续释放和现场递送来封装用于治疗局部疾病的药物。我们假设CS/HA NGS可以共负载MTX和ALA,同时促进MTX和ALA在皮肤中的渗透,从而达到治疗银屑病的目的。

    在这里,我们首次制备了用于MTX和ALA同步递送的CS/HA NGS(MTX-ALA NGS)。体外和体内渗透实验表明,NGS能促进MTX和ALA在表皮中的渗透,并使表皮转化更多的PpIX。细胞对ALA和MTX的摄取同时增加,产生更多的PpIX和ROS,从而抑制脂多糖(LPS)刺激的HaCaT细胞的增殖并诱导其凋亡。对咪喹莫特(imiquimod,ImQ)诱导的小鼠体内抗银屑病作用进行了观察。总体结果表明,CS-HA NG联合MTX和ALA治疗银屑病是一种很有前途的联合化疗-光动力疗法。

    与甲氨蝶呤-丙氨酸混悬剂相比,该制剂在体内外均能促进甲氨蝶呤和丙氨酸的透皮吸收和滞留(p<0.001)。NGS可促进细胞摄取(p<0.001)、原卟啉IX转化率(p<0.001)和活性氧生成(p<0.001),对脂多糖刺激的HaCaT细胞具有协同的抗增殖和凋亡作用,凋亡率为78.6%。MTX-Ala NGS能有效改善咪喹莫特诱导的银屑病小鼠的皮肤症状,下调促炎症细胞因子TNF-α和IL-17A(p<0.001)。重要的是,MTX-Ala NGS降低了口服MTX对肝脏和肾脏的毒性。

     虽然口服MTX和外用ALA-PDT是治疗银屑病的两种传统疗法,但MTX的全身毒性和ALA的渗透性差限制了它们的使用。在这项研究中,我们首次开发了一种自组装的CS-HA纳米凝胶,将MTX和ALA用于局部联合化疗/光动力疗法,从而将副作用降至最低,并提高了银屑病的治疗效果。NGS同时增加MTX和ALA的皮肤通透性。体外细胞研究表明,NGS增强了细胞摄取,改善了抗增殖和凋亡活性。外用MTX-Ala NGS可显著提高对ImQ诱导的BALB/c小鼠银屑病样斑的治疗效果,降低口服MTX对肝、肾的全身毒性。综上所述,我们的研究表明CS-HA NGS可以共负载MTX和ALA,同时促进MTX和ALA在皮肤中的渗透,从而对银屑病进行化学-光动力学联合治疗。
